摘要

目的:探讨血府逐瘀汤加减方对脑卒中后抑郁患者自主神经功能及生活质量的影响。方法:选取2017年1月至2019年5月在新疆医科大学第一附属医院治疗的90例脑卒中后抑郁患者

按入院顺序编号

以随机数字表法分为对照组和观察组

每组45例。对照组患者采用文拉法辛口服治疗

观察组患者在对照组的基础上加用血府逐瘀汤加减方水煎服治疗。观察两组患者的疗效

比较治疗前和治疗后3个月的抑郁程度[汉密尔顿抑郁量表（hamilton depression scale

HAMD）评分]、神经功能缺损程度[美国国立卫生院卒中神经功能缺损评分量表（national institutes of health stroke scale

NIHSS）评分]、自主神经功能[交感神经皮肤反应（SSR）和R-R间期变化率（RRIV）]、神经细胞因子[神经生长因子（NGF）、脑源性神经营养因子（BDNF）]水平、单胺类递质[去甲肾上腺素（NE）、5-羟色胺（5-HT）]水平及生活质量情况[总体幸福感指数量表（general well-being schedule

GWB）评分、Barthel指数]。结果:观察组患者的总有效率为93.33%（42/45）

明显高于对照组的77.78%（35/45）

差异有统计学意义（P <0.05）。观察组患者治疗后的HAMD评分、NIHSS评分

上肢SSR、下肢SSR、安静RRIV及深呼吸RRIV水平明显低于对照组;NGF、BNDF、5-HT及NE水平

GWB评分、Barthel指数明显高于对照组

上述差异均有统计学意义（P<0.05）。结论:血府逐瘀汤加减方能有效降低脑卒中后抑郁患者的抑郁程度

改善其自主神经功能

提高日常生活质量。

Abstract

OBJECTIVE: To probe into the effect of modified Xuefuzhuyu decoction on autonomic nervous function and quality of life in patients with post-stroke depression. METHODS: Totally 90 patients with post-stroke depression admitted into the First Affiliated Hospital of Xinjiang Medical University from Jan. 2017 to May 2019 were selected

according to the admission sequences

these patients were divided into control group and observation group via random number table

with 45 cases in each group. The control group was treated with venlafaxine

while the observation group was given modified Xuefuzhuyu decoction based on the control group. The efficacy of both groups were observed

differences in depression degree [hamilton depression scale （HAMD） score]

neurologic impairment [national institutes of health stroke scale （NIHSS） score ]

autonomic nervous function [sympathetic skin response （SSR） and R-R interphase variation （RRIV） ]

neurocytokine [nerve growth factor （NGF）

brain-derived neurotrophic factor （BDNF） ]

monoamine transmitters [norepinephrine （NE）

5-hydroxytryptamine （5-HT） ]and quality of life [general well-being schedule （GWB） score

Barthel index] were compared between two groups before treatment and 3 months after. RESULTS: The total effective rate of observation group was 93. 33% （42/45）

which was significantly higher than that of the control group （77. 78%

35/45）

improve their autonomic nervous function and promote the quality of daily life.
