Review and Analysis of 1454 Prescriptions of Antibiotics for Urinary Tract Infection in Outpatient and Emergency Department
OBJECTIVE:To investigate the application of antibiotics for urinary tract infection in outpatient and emergency department in Beijing Changping District Hospital of Integrated Traditional Chinese and Western Medicine(hereinafter referred to as "our hospital")
so as to provide reference for the rational application of antibiotics.METHODS:Retrospective analysis was used
from Jul.2018 to Dec.2019
1 454 prescriptions of antibiotics for urinary tract infection in the outpatient and emergency department of our hospital were randomly selected for review and analysis.Excel 2017 software was used for statistical analysis
and Pareto chart analysis was performed on part of the contents.RESULTS:Among the 1 454 prescriptions of antibiotics for urinary tract infection
1 382 were rational prescriptions
and the rational rate was 95.05%.The irrational rate of prescriptions from Jul.to Dec.2018
from Jan.to Jun.2019 and from Jul.to Dec.2019 were respectively 7.65%(42/549)
3.74%(14/374) and 3.01%(16/531)
showing the decreasing trend. The proportion of prescriptions for drug combination of antibiotics decreased from7.65%(42/549) from Jul.to Dec.2018 to 3.95%(21/531) from Jul.and Dec.2019.Application of oral antibiotics accounted for 80%
and restricted antibiotics accounted for 50%.The proportion of aminoglycosides dropped from7.83%(43/549) from Jul.to Dec.2018 to 2.64%(14/531) from Jul.to Dec.2019.There were 72 irrational prescriptions
of which 70 prescriptions(75 case-times) were inappropriate prescriptions
including 17 case-times of inappropriate indications
14 case-times of inappropriate selection of drugs
8 case-times of improper dosage forms or route of administration
31 case-times of improper usage and dosage
and 5 case-times of improper drug combination.CONCLUSIONS:The prescription-review project has promoted the rational application of antibiotics for urinary tract infection in our hospital. The prescription of drug combination has been significantly reduced
and the application of aminoglycosides has become more standardized
yet problems such as excessive use of antibiotics still need the continuous improvement.
