摘要

目的:探讨重组人血管内皮抑制素联合洛铂胸腔灌注治疗恶性胸腔积液的疗效及对恶性胸腔积液血管内皮生长因子（VEGF）、癌胚抗原（CEA）、免疫状态及预后的影响。方法:选取2013年1月至2018年12月江苏盛泽医院收治的恶性胸腔积液患者60例

根据治疗方法不同分为观察组和对照组

每组30例。所有患者均行胸腔穿刺置管

充分引流胸水排尽胸腔积液;观察组患者采用重组人血管内皮抑制素60 mg（1周2次

连续2周）+洛铂30 mg/m

（1周1次

连续2周）行胸腔灌注化疗;对照组患者采用单纯洛铂30 mg/m

行胸腔灌注化疗

1周1次

连续2周。观察两组患者的临床疗效、生活质量和不良反应发生情况

比较两组患者胸腔积液VEGF、CEA水平

外周血淋巴细胞亚群及免疫球蛋白[免疫球蛋白A（IgA）、免疫球蛋白G（IgG）及免疫球蛋白M（IgM）]水平。结果:治疗后7 d

两组患者胸腔积液VEGF和CEA水平明显降低

外周血CD

、CD

/CD

、IgG和IgA水平明显升高

水平明显降低

差异均有统计学意义（P

0.05）;与对照组比较

观察组患者胸腔积液VEGF和CEA水平明显降低

外周血细胞亚群及免疫球蛋白水平明显改善

差异均有统计学意义（P

0.05）;但两组患者IgM水平的差异无统计学意义（P

0.05）。观察组患者的总有效率为63.33%（19/30）

明显高于对照组的36.67%（11/30）

差异有统计学意义（P

0.05）。观察组患者生活质量改善率明显高于对照组

生活质量降低率明显低于对照组

无进展生存期明显长于对照组

差异均有统计学意义（P

0.05）。两组患者不良反应发生率的差异无统计学意义（P

0.05）。结论:重组人血管内皮抑制素联合洛铂治疗恶性胸腔积液具有协同作用

患者生活质量明显改善

不良反应较轻

其机制与降低胸腔积液VEGF和CEA水平及改善患者免疫功能状态有关。

Abstract

OBJECTIVE: To probe into the efficacy of recombinant human endostatin combined with loboplatin in the treatment of malignant thoracic fluid and its effects on vascular endothelial growth factor（ VEGF） and carcinoembryonic antigen（ CEA）

immune status and prognosis.METHODS: Totally 60 patients with malignant thoracic fluid admitted into Jiangsu Shengze Hospital from Jan.2013 to Dec.2018 were extracted to be divided into the observation group and the control group according to the different treatment schemes

with 60 cases in each group.All patients were given chest cavity puncture tubes

fully drained chest water drain edgy fluid.Patients in the observation group were treated with recombinant human endostatin 60 mg（ twice a week for 2 weeks） + loboplatin 30 mg/m

（ once a week for 2 weeks） for intrapleural chemotherapy.The control group received thoracic fluid chemotherapy with loproplatin 30 mg/m

once a week for 2 weeks.The clinical efficacy

quanlity of life and adverse drug reactions of two groups were observed.The levels of thoracic fluid VEGF and CEA

the levels of peripheral blood lymphocyte subsets and immunoglobulin[immunoglobulin A（ IgA）

immunoglobulin G（ IgG） and immunoglobulin M（ IgM） ] of two groups were compared.RESULTS: After treatment of 7 d

the levels of VEGF and CEA in thoracic fluid of two groups decreased significantly

and the levels of CD3

CD4

/CD8

IgG and IgA in the peripheral blood increased significantly

and the levels of CD8

decreased significantly

the differences were statistically significant（ P

0.05）.Compared with the control group

the levels of VEGF and CEA in thoracic fluid in the observation group reduced significantly

and the peripheral blood lymphocyte subsets and immunoglobulin were significantly improved

with statistically significant differences（ P

0.05）.However

there was no significant difference in IgM between two groups（ P

0.05）.The total effective rate of the observation group was 63.33%（ 19/30）

significantly higher than that of the control group 36.67%（ 11/30）

the difference was statistically significant（ P

0.05）.The improvement rate of the quality of life of the observation group was significantly higher than that of the control group

the decline rate of the quality of life was significantly lower than that of the control group

and the PFS was significantly longer than that of the control group

the differences were statistically significant（ P

0.05）.The difference in the incidence of adverse drug reactions between two groups was not statistically significant（ P

0.05）.CONCLUSIONS: Recombinant human endostatin combined with lobaplatin in the treatment of malignant thoracic fluid has synergy effects

with improved quality of life

few adverse drug reactions

reduced thoracic fluid VEGF and CEA levels and improved immune function.
