Efficacy and Safety of Different Doses of Lidocaine Intravenous Infusion on Postherpetic Neuralgia and Its Effects on Dosage of Analgesic Drugs
OBJECTIVE: To probe into the clinical efficacy and safety of different doses of lidocaine intravenous infusion on postherpetic neuralgia and its effects on dosage of analgesic drugs.METHODS: Totally 180 patients with postherpetic neuralgia diagnosed in the pain department of the Second Affiliated Hospital of Army Military Medical University from Jan.2018 to Jun.2019 were selected to be divided into the conventional group
the lidocaine group A and the lidocaine group B via the random number table
with 60 cases in each group. The conventional group were treated with conventional drugs
the lidocaine group A were given intravenous infusion of 0.1% lidocaine 100 ml/d on the basis of conventional drugs
and the lidocaine B group received intravenous infusion of 0.1% lidocaine 200 ml/d on the basis of conventional drugs.Pain status [visual analogue scale( VAS) ]
sleep status [quality of sleep score( QS) ]
negative mood status [Hamilton anxiety scale( HAMA) and Hamilton depression scale( HAMD) ]
the application of analgesics and adverse drug reactions were observed in admission( D
0
)
the treatment day 1( D
1
)
the treatment day 3( D
3
) and the treatment day 7( D
7
).RESULTS: The VAS score and QS score of lidocaine group A and lidocaine group B were significantly lower than those of the conventional group in D
1
D
3
and D
7
the difference was statistically significant( P
<
0.05).The VAS score in D
7
and QS score in D
1
D
3
and D
7
of lidocaine group B were significantly lower than those of lidocaine group A
the difference was statistically significant( P

D
3
and D
7
and HAMD score in D
3
and D
7
of lidocaine group A and lidocaine group B were significantly lower than those of the conventional group( P
<
0.05)
and the HAMA score and HAMD score in D
7
of lidocaine group B were significantly lower than those of lidocaine group A
with statistically significant differences( P
<
0.05).The doses of oxycodone and acetaminophen in D
3
and D
7
and tramadol in D
7
in lidocaine group A and lidocaine group B in D
3
and D
7
were significantly lower than those in the conventional group
the differences were statistically significant( P
<
0.05); the doses of tramadol in D
3
in lidocaine group B were significantly lower than that in the conventional group
with statistically significant difference( P
<
0.05)
and the doses of oxycodone and acetaminophen in D
3
and D
7
and tramadol in D
7
in lidocaine group B were significantly lower than those in lidocaine group A
with statistically significant differences( P
<
0.05).None of the patients in three groups could not complete the treatment due to severe adverse drug reactions
and the difference in the total incidence of adverse drug reactions among three groups was not statistically significant( P
>
0.05).CONCLUSIONS: Lidocaine has significant clinical efficacy in the treatment of postherpetic neuralgia
and can reduce the dosage of tramadol and other analgesic drugs
without increasing the adverse drug reactions and the safety is higher. Moreover
