摘要

目的:探讨不同种类餐时胰岛素联合基础胰岛素治疗2型糖尿病的有效性和经济性。方法:采用回顾性研究方法

选择2016年1月至2019年1月天津医科大学朱宪彝纪念医院收治的216例确诊为2型糖尿病的患者资料

根据治疗方案不同分为A组（56例）、B组（53例）、C组（54例）及D组（53例）。A组患者采用赖脯胰岛素+甘精胰岛素治疗

B组患者采用赖脯胰岛素+地特胰岛素治疗

C组患者采用门冬胰岛素+地特胰岛素治疗

D组患者采用门冬胰岛素+甘精胰岛素治疗。四组患者的疗程均为2周。比较四组患者治疗前后的血糖水平和临床疗效

并采用成本-效果分析法进行经济学评价。结果:A、B、C及D组患者的总有效率分别为91. 07%（51/56）、84. 91%（45/53）、87. 04%（47/54）及86. 79%（46/53）

差异无统计学意义（P>0. 05）。A、B、C及D组方案的成本分别为320. 24、327. 06、331. 16及324. 31元;成本-效果比分别为3. 52、3. 85、3. 80及3. 74

A组最低; B、C及D组方案的增量成本-效果比为-1. 11、-2. 71及-0. 95

A组方案更具有成本-效果优势。敏感度分析结果支持成本-效果分析结果。结论:从短期疗效上分析

赖脯胰岛素联合甘精胰岛素治疗2型糖尿病的方案更优。

Abstract

OBJECTIVE: To probe into the effectiveness and economy of different types of meal and basic insulin therapy for type 2 diabetes. METHODS: Retrospective analysis was adopted

totally 216 patients with type 2 diabetes admitted into Tianjin Medical University Chu Hisen-I Memorial Hospital from Jan. 2016 to Jan. 2019 were extracted to be divided into group A（n = 56）

group B（n = 53）

group C（n = 54） and group D（n = 53） according to different treatment regimen. Group A was treated with insulin lispro + insulin glargine

group B was treated with insulin lispro +insulin detemir

group C was treated with insulin aspart+insulin detemir

and group D was treated with insulin aspart+insulin glargine. The treatment course of four groups was 2 weeks. The blood glucose level before and after treatment and clinical efficacy were compared among four groups. And the cost-effectiveness analysis was used for economic evaluation. RESULTS: The total effective rate of group A

C and D was 91. 07%（51/56）

and group A had more cost-effectiveness advantages. Sensitivity analysis results supported cost-effectiveness analysis results. CONCLUSIONS: From the efficacy analysis of short-term treatment

the treatment of insulin lispro combined with insulin glargine for type 2 diabetes is better.
