摘要

目的:了解复旦大学附属华东医院（以下简称"该院"）老年冠心病住院患者使用替格瑞洛的合理性及安全性

为临床合理用药提供参考。方法:采用回顾性调查方法

对2018年6月至2019年9月该院162例使用替格瑞洛的老年冠心病住院患者进行用药合理性评价与分析。结果:162例使用替格瑞洛的老年冠心病住院患者中

161例（占99. 38%）的适应证符合相关指南的推荐;临床用药过程中

患者多病共存和多重用药现象较为普遍

且出现了出血（36例

占22. 22%）

血尿酸升高（18例

占11. 11%）和心动过缓（4例

占2. 47%）等症状;替格瑞洛与活血化瘀类中成药联合应用

可增加患者的出血不良反应。建议用药期间加强临床监测

确保用药安全。结论:由于替格瑞洛在国内临床使用经验有限

且没有特异性拮抗剂

临床应用的药效学和安全性有待进一步验证。

Abstract

OBJECTIVE: To investigate the application rationality and safety of ticagrelor in elderly patients with coronary heart disease in Huadong Hospital Affiliated to Fudan University（ hereinafter referred to as "this hospital"）

so as to provide reference for the rational drug use in clinic. METHODS: Retrospective investigation method was adopted

evaluation and analysis were conducted on application rationality of ticagrelor in 162 cases of elderly patients with coronary heart disease from Jun. 2018 to Sept. 2019 in this hospital. RESULTS: Of the 162 cases of elderly patients with coronary heart disease

the indications of 161 cases（ 99. 38%） met the recommendations of relevant guidelines. During the course of clinical medication

the coexistence of multiple diseases and multiple drugs were common

and the patients presented bleeding（ 36 cases

22. 22%）

elevated uric acid（ 18 cases

11. 11%）

bradycardia（ 4 cases

2. 47%） and other symptoms. The combination of ticagrelor and traditional Chinese medicines for promoting blood circulation and removing blood stasis can increase adverse drug reactions of bleeding in patients. It is recommended to strengthen clinical monitoring during medication

so as to ensure medication safety.CONCLUSIONS: Due to limited clinical experience in China and the absence of specific antagonists

the pharmacodynamics and safety of clinical application need to be further verified.
