Symptom Characteristics and Related Factors of Mental Disorders Induced by Voriconazole
OBJECTIVE: To investigate the symptom characteristics and related factors of mental disorders induced by voriconazole
so as to summarize the clinical prevention and treatment strategies. METHODS: Retrospective analysis was conducted on 386 patients with fungal infection treated with voriconazole in Nanping First Hospital Affiliated to Fujian Medical University from Aug. 2010 to May 2018. According to the occurrence of mental disorders
all patients were divided into the occurrence group and the non-occurrence group. The clinical symptoms of patients with mental disorders were sorted out
and the clinical data of the occurrence group and the non-occurrence group were compared.Multivariate unconditional Logistic regression analysis was used to summarize the related factors affecting the mental disorders induced by voriconazole. RESULTS: Of the 386 patients
62 cases had mental disorders during voriconazole administration
with the incidence of 16. 06%. The mental disorders of the patients occurred on( 3. 06±0. 88) d after the administration of voriconazole. The main clinical symptoms were consciousness disorder
followed by sensory disorder and affective disorder. The psychiatric symptoms of the patients improved or disappeared on( 1. 97±0. 37) d after the withdrawal of voriconazole. Multivariate regression analysis showed that age( ≥60 years old)
sustained dose of voriconazole [≥ 6 mg/( kg·d) ]
serum alanine aminotransferase( ≥ 35 U/L) and serum total bilirubin( ≥17. 1 μmol/L) were independent risk factors for mental disorders induced by voriconazole( P < 0. 05).CONCLUSIONS: Voriconazole-induced mental disorders are not uncommon. Although patients with mental disorders can be significantly improved in the short term after drug withdrawal
consciousness disorders and sensory perception disorders induced by voriconazole may also have adverse effects on the prognosis of patients. Therefore
monitoring
prevention and treatment of mental disorders in high-risk patients should be strengthened.
