摘要

目的:探讨单硝酸异山梨醇酯联合培哚普利治疗慢性心力衰竭的临床疗效及对血浆N末端B型钠尿肽原（NT-proBNP）的影响。方法:选取2017年12月至2018年12月安徽医科大学附属阜阳医院收治的慢性心力衰竭住院患者96例

采用随机数字表法分为观察组和对照组

每组48例。在常规治疗基础上

对照组患者采用培哚普利治疗

观察组患者采用单硝酸异山梨醇酯联合培哚普利治疗。比较两组患者的每搏输出量（SV）、每分钟心排血量（CO）、左心室射血分数（LVEF）、左心室舒张末期内径（LVEDD）及左心室舒张早期血流峰值速度/舒张末期血流峰值速度的比值（E/A）等指标水平

观察两组患者的治疗效果及血浆NT-proBNP水平变化情况。结果:治疗后

两组患者的SV、CO、LVEF及E/A水平均明显高于治疗前

LVEDD水平明显低于治疗前

且观察组患者上述指标水平明显优于对照组

差异均有统计学意义（P<0. 05）。观察组患者的总有效率为91. 67%（44/48）

明显高于对照组的72. 92%（35/48）

差异有统计学意义（P<0. 05）。治疗后

两组患者的血浆NT-proBNP水平均明显低于治疗前

且观察组患者明显低于对照组

差异均有统计学意义（P<0. 05）。结论:相较于单一使用培哚普利

单硝酸异山梨醇酯联合培哚普利治疗慢性心力衰竭

能显著改善患者心功能指标水平

降低血浆NT-proBNP水平

利于患者康复。

Abstract

OBJECTIVE: To probe into the efficacy of isosorbide mononitrate combined with perindopril in the treatment of chronic heart failure and its effects on N-terminal B-type natriuretic peptide（ NT-proBNP）. METHODS:Totally 96 patients with chronic heart failure admitted into Fuyang Hospital

Anhui Medical University from Dec. 2017 to Dec. 2018 were extracted to be divided into the observation group and the control group via the random number table

with 48 cases in each group. Based on conventional treatment

the control group was treated with perindopril

while the observation group received isosorbide mononitrate combined with perindopril. The stroke volume（ SV）

cardiac output per minute（ CO）

left ventricular ejection fraction（ LVEF）

left ventricular end diastolic diameter（ LVEDD）

and left ventricular peak diastolic blood velocity/end diastolic peak blood velocity ratio（ E/A） of two groups were compared. The clinical efficacy and changes of plasma NT-proBNP levels were observed. RESULTS: After treatment

the levels of SV

LVEF and E/A of two groups were significantly higher than those before treatment

and the level of LVEDD was significantly lower than that before treatment

and the levels of the above indicators in the observation group were significantly better than those in the control group

the differences were statistically significant（ P<0. 05）. The total effective rate in the observation group was 91. 67%（ 44/48）

significantly higher than that in the control group（ 72. 92%

35/48）

with statistically significant difference（ P < 0. 05）. After treatment

the plasma NT-proBNP levels in two groups were significantly lower than those before treatment

the observation group was significantly lower than the control group

and the differences were statistically significant（ P < 0. 05）.CONCLUSIONS: Compared with perindopril alone

isosorbide mononitrate combined with perindopril in the treatment of chronic heart failure can significantly improve patient’s cardiac function

reduce the plasma NT-proBNP levels

which is conducive to rehabilitation.
