Application of Voriconazole in Patients with Malignant Hematological Disease and Invasive Fungal Infection
OBJECTIVE: To probe into the application status of voriconazole in patients with malignant hematological disease and invasive fungal infection
so as to provide reference for clinical rational drug application.METHODS: The electronic medical record system and the hospital information management system were used to retrospectively collect the medical records of voriconazole in patients with malignant hematological disease and invasive fungal infection in a grade A class three hospital in Tianjin from May to Dec. 2018. Case inclusion was performed according to inclusion and exclusion criteria. Epidate software was used for data processing. SPSS 22. 0 software was adopted for statistical analysis. RESULTS: Totally 378 patients were included. In terms of medication purpose
voriconazole was mainly used for prevention and diagnosis-driven therapy. A total of 305 patients were tested for fungi
and the rate of fungal screening was 80. 69%
the positive rate was 3. 93%( 12/305). A total of 289 patients had irrational application of voriconazole
and the irrational rate reached 76. 46%
the main irrational reason was that no loading dose was given. In diagnosis-driven therapy
there was no correlation between the loading dose and the effectiveness of voriconazole therapy( P>0. 05). The increase of total drug cost was an important factor for the increase of hospitalization cost
and the high cost of voriconazole may be an important reason for the increase of total drug cost.CONCLUSIONS: The unreasonable use of voriconazole in patients with malignant hematopathy and invasive fungal infection is serious
clinicians should pay attention to the problem of whether to give the load dose during the use of voriconazole
and control the drug cost reasonably and effectively.
