摘要

目的:比较谷氨酰转肽酶（γ-glutamyl trasnpeptidase

GGT）-H亚型和甲胎蛋白（Alpha-fetoprotein

AFP）检测对评估原发性肝癌（Hepatocellular carcinoma

HCC）根治术后生存的价值。方法:67例HCC患者于手术当日晨起空腹抽外周静脉血10ml

应用PCR法检测GGT mRNA-H亚型的表达

并采用放射免疫测定法检测AFP水平

评估两者对评估HCC根治术后生存的预测价值。结果6:7例患者1、3、5年累积生存率分别为71.5%、35.1%和18.7%

其中GGT mRNA-H亚型表达阴性者为79.5%、42.3%和22.1%

而表达阳性者为65.5%、24.1%和11.7%（χ2=3.014

P=0.021）。而AFP<400μg/L的HCC患者术后1、3、5年累积生存率分别为76.3%、44.6%和23.9%

AFP>400μg/L的1年、3年、5年累积生存率分别为62.1%、21.1%和13.2%（χ2=2.697

P=0.035）。Cox模型显示GGT mRNA-H对预后的影响也较AFP大（OR=1.672 vs OR=1.145）。结论:外周血细胞GGTmRNA-H亚型表达的检测是评估HCC术后预后的可靠指标

并要优于AFP的检测。

Abstract

Objective:To evaluate the relationship between GGT mRNA-H subtype expression and the survival of HCC patients after radical mastectomy.Methods:67 patients with HCC were enrolled in this study.On the surgery day

10 ml peripheral venous blood was drawn from each patient in the morning

PCR used to detect the GGT mRNA-H subtype expression.AFP level were also detected by radioimmunoassay.Results:The 1

5-year survival rates of the 67 HCC patients were 71.5%

35.1% and 18.7% respectively

of which 79.5%

42.3% and 22.1% were found respectively in the negative expression of GGT mRNA-H subtype

and 65.5%

24.1% and 11.7% found respectively in the positive expression(χ2=3.014

P=0.021).Cox model showed that GGT mRNA-H had more impact on prognosis than the AFP(OR=1.672 vs OR=1.145).Conclusion:Peripheral blood cell GGTmRNA-H subtype expression detection is a reliable indicator to predict the prognosis of primary liver cancer patients

and it’s better than AFP testing.
