Clinical and pathological patterns of infratentorial brain tumors in children
Objective:To explore the experience of the diagnosis and treatment of infratentorial tumors in children
in order to make early diagnosis
get reasonable treatment and improve prognosis.Methods:91 cases from July 2003 to December 2009 with infratentorial tumors admitted into children’s hospital of Chongqing medical university were analyzed retrospectively.Results:29 cases(31.8%)were misdiagnosed. The top three of tumors were medulloblastoma(33 cases
36.3%)
astrocytoma(28 cases
30.8%)
and ependymoma(21 cases
23.1%).The main clinical manifestations were vomiting
headache
and incoordination.Obstructive hydrencephalus was found in 71 cases(78.4%) by imaging examination.Positive rate of tumors diagnosis was 94.2% and 100% by brain CT and MRI scanning
respectively.Total resection was achieved in 81 cases(89.01%);subtotal resection in 6 cases(6.6%);partial resection in 4 cases(4.39%)
and clinical cure in 84 cases. After operation
41 received radiotherapy(45.0%)and 30 received chemotherapy(32.9%).Conclusion:Children’s infratentorial tumors have atypical clinical manifestations and short course of diseases and hydrocephalu. It is common to be misdiagnosed.Brain MRI and CT are the initial choice in the diagnosis.Surgical resection is the first selection to cure intracranial tumor in childhood.Positive treatment of hydrocephalus and postoperative complications is beneficial to improve the surgical outcome. After operation
individual radiotherapy and chemotherapy are helpful for longer survival.
