摘要

目的:探讨包括手术、放疗、化疗、生物免疫学治疗在内的综合治疗对于滑膜肉瘤（Synovial sarcoma

SS）的临床疗效。方法:随机选择79例自2000-2010年在我院经过综合治疗的SS患者

对其临床疗效和预后进行比较、分析和评估。结果:随访3

0年（平均6年）

发现经过手术结合放疗、化疗、生物免疫学治疗等综合治疗的患者3、5年复发及转移率:四肢组（60例）分别为28.3%、41.7%

躯干组（19例）为0、10.5%;四肢组复发及转移率明显高于躯干组。3年总无瘤生存率78.5%

总有效生存率为89.9%;5年总无瘤生存率65.8%

总有效生存率为75.9%。结论:患者就诊的早晚和肿瘤所在部位对预后影响极大。早期作出正确的诊断

积极采取对肿瘤广泛深入切除

区域肿大淋巴结清除

结合术前术后放、化疗及免疫生物学治疗的综合治疗方法

是提高治疗效果的关健。

Abstract

Objective:To approach clinical curative effects of combined therapy including operation

radiotherapy

chemotherapy

and immunobiologic therapy for synovial sarcoma.Methods:79 cases of synovial sarcoma with the combined therapy from 2000 to 2010 were randomly selected to evaluate the clinical curative effects and their prognosis.Results:The mean follow-up time was 6 years(3~10 years).Combined therapies included operation

radiotherapy

chemotherapy

and immunobiologic therapy.The rates of regional recurrence and metabasis were as follows:28.3% and 41.7% respectively in limbs group(60 cases);0% and 10.5% respectively in trunk group(19 cases).The rates of regional recurrence and metabasis of limbs group were higher than those of trunk group.The rate of survival without tumor for 3 years was 78.5%;the rate of general effective survival was 89.9%;those for 5 years were 65.8% and 75.9% respectively.Conclusion:The time of diagnosis and treatment and the location of the sarcoma are very important to their prognosis.To make a correct diagnosis and to give a wide and thorough removal in the cleaning of regional lymphadenectasis in early stage and the combined therapies including radiotherapy

chemotherapy

and immunobiologic therapy pre-and post-operation is the key to the improvement of the clinical curative effect.
