摘要

目的:分析幕上高血压性脑出血患者死亡和神经功能恢复的影响因素。方法:回顾性分析270例幕上高血压性脑出血病例的相关临床、头颅CT及实验室检查资料

并随访出血后90 d的神经功能（MRS评分）。采用多因素Logistic回归方法分析所选相关变量和出血后30 d死亡及90 d神经功能恢复的关系。结果:在本组270例患者中

幕上高血压性脑出血后30 d死亡者为53例（19.6%）9

0 d神经功能恢复者为79例（29.3%）。入院时意识障碍（OR=0.233

P=0.024）、血肿体积（OR=1.021

P=0.043）。破入脑室（OR=0.478

P=0.048）及高血糖（OR=0.194

P=0.000 4）是出血后30 d死亡的影响因素。年龄（OR=4.947

P=0.016 8）是出血后90d神经功能恢复的影响因素。结论:入院时意识、血肿体积、破入脑室及血糖是幕上高血压性脑出血后30 d死亡的影响因素;年龄、入院时意识、血肿部位、血肿体积、破入脑室、白细胞计数、ALT是出血后90 d神经功能恢复的影响因素。以上因素对判断患者的预后及指导临床治疗有一定的意义。

Abstract

Objective:To discuss the influencing factors of mortality and functional recovery in patients with supratentorial hypertensive intracerebral hemorrhage(ICH).Methods: Retrospective analyzed the 270 consecutive cases with supratentorial hypertensive ICH admitted in department of Neurology and neurosurgery

with relevant clinical

computerized tomography(CT)and laboratory parameters material.The patients were followed-up bleeding after 90 days functional recovery after ICH(Score evaluation by MRS).Using multivariate logistic regression analysis

the associations between all selected variables and 30-day mortality and 90-day functional recovery after ICH were evaluated.Results:30-day mortality was achieved in 19.6%(n=53)of the 270 patients 90-day functional recovery in 29.3%(n=79);initial unconsciousness(OR=0.233
