The effect of intensie insulin therapy on immune status for polytrauma patients
Objective:To explore the effect of intensie insulin therapy on immune status of polytrauma patients in ICU.Methods:Fifty-four polytrauma patients in ICU were randomly divided into two groups
the insulin intensive therapy group(I group)(n=28) and control group(M group)(n =26).The blood glucose in the I group was controlled at 4.4 to 6.1 mmol/L
and the blood glucose in M group was controlled at 10.0 to 11.1 mmol/L.The two groups were observed and compared the CRP
IL-6 and TNF-αin serum before therapy and after therapy 1
3
5 d
also compared the mortality.Results:The CRP
IL-6 and TNF-αin serum is more higher than normal before therapy in all the patients.Serum CRP
IL-6 in the first five days after therapy were significantly lower in I group group than those in M group(P<0.05)
TNF-αin the first days after therapy were significantly lower in the I group than those in M group(P<0.05)
and then has obvious trend to fallen in the first five days after treatment(P<0.05).Conclusion:Intensive insulin therapy and keeping blood glucose at 4.4 to 6.1 mmol/L can reduce the mortality and CRP
IL-6 and TNF-α in serum for the critically polytrauma patients patients.
