Impact of intravenous immunoglobulin on the panel reactive antibody in renal transplantation
Objective:To investigate the effect of intravenous immunoglobulin(IVIG)on the panel reactive antibody(PRA)in renal transplantation.Methods:57 highly sensitized patients awaiting renal transplantation
age 21 to 65 years
the value of PRA were 35% to 91%(mean46.7%±29.5%).All received the treatment by IVIG 5g/d
iv
gtt
period of treatment was 2 weeks. We ex- amined the value of PRA before treatment and after treatment of IVIG.Results:36 patients used one period of treatment
15 patients used two period of treatment
6 patients used three period of treatment. After treatment of IVIG
the value of PRA de- creased to non-sensitivity in 48 patients
completely no decreased in 3 patients
partly decreased in 6 patients. The patients re- sulted in reduction in PRA of 0~61%(mean decrease:38.4%±16.3%)
there were significant difference in patients with treat- ment of IVIG.All patients received renal transplantation which CDC less than 10%
there were no hyperacute rejection ap- pearance. After operation
there were 2 patients(3.5%)occurred acute rejection and treated with MP plus FK506
the renal function returned normal gradually. Conclusion:Intravenous injection of high dose of IVIG could reduce the value of PRA in some highly sensitized patients and could increased the rate of succeed in renal transplantation.
