摘要

目的:探讨不同方法检测耐甲氧西林凝固酶阴性葡萄球菌（MRCNS）的敏感性

并对其耐药性进行分析。方法:应用PCR检测mecA基因、E-test、青霉素结合蛋白2a（PBP2a）乳胶凝集法、苯唑西林（OXA）琼脂筛选法和K-B法对临床分离的200株凝固酶阴性葡萄（CNS）球菌进行MRCNS检测

并用WHONET4软件进行耐药性分析。结果:各方法MRCNS检测阳性率分别为:OXA琼脂筛选法74%;OXAK-B法72%;E-test71%;PBP2a乳胶凝集法73.5%

mecA基因检测法70%

检测结果无显著性差异（χ2=1.5124

P>0.05）。MRCNS对青霉素（PEN）、庆大霉素（GEN）、复方磺胺（SXT）、环丙沙星（CIP）、四环素（TET）、克林霉素（CLI）、红霉素（ERY）等传统抗生素耐药率均在50%以上;甲氧西林敏感凝固酶阴性的葡萄球菌（MSCNS）除PEN、ERY耐药率为80.4%和57.1%以外

其他抗生素均小于或在50%左右

CNS中耐甲氧西林葡萄球菌（MRS）达70%以上

未发现耐万古霉素（VAN）的CNS。结论:五种方法均能满足MRCNS的临床筛选

其中以乳胶凝集法最为简便、快速

临床值得推广;MRCNS对氨基甙类、喹诺酮类、四环素类、大环内酯类、磺胺存在严重的交叉耐药现象

经验用药首选VAN

而MSCNS除VAN外

还可选用GEN、SXT、CIP、TET、CLI。

Abstract

Objective:To investigate the sensibility of different methods to test MRCNS

and analyze the antibiotic resistance of CNS.Methods:200 strains CNS were detected by mecA genic

E-test

PBP2a latex agglutination

OXA agar election and K-B

and analyze the resistance with WHONET4.Results:The positive rates of MRCNS by agar election method

K-B

E-test

PBP2a latex agglutination

mecA genic were 70%

74%

72%

71%

73.5%respectively.There is no statistic significance in 5 assays(χ2=1.5124

P>0.5).The resistance rates of MRCNS to PEN

GEN

SXT

CIP

TET

ALL and ERY

are all above 50%.The resistant rates of MSCNS to other antibiotics are below about 50% except for PEN(80.4%)and ERY(57.1%).MRS are more than 70% and no VAN resistant in CNS.Conclusion:All the 5 methods are suitable for clinical detection of MRCNS

and latex agglutination is the most convenient and rapid.MRCNS had a serious cross-resistance in aminoglycosides

guinolones

tetracycline

macrolides

sulfanilamide and

so the best choice for therapy is VAN.GEN

SXT

CIP

TET and CLI could be used for MSCNS

except VAN.
