摘要

目的 :研究胸腔镜下肺大疱缝扎及结扎法治疗自发性气胸的临床效果。方法 :选择电视胸腔镜（VATS）下使用普通胸外科的缝针及缝线肺大疱缝扎及结扎法治疗自发性气胸 13例

随访 1～ 12个月

对手术时间、胸管引流时间、胸管引流量、术后 2 4h停用强止疼药者（% ）、平均住院日、住院总费用和药费进行分析。结果 :手术效果满意

无并发症、死亡及术后复发

平均住院日 3天

临床效果明显好于腋下小切口手术治疗自发性气胸。住院总费用（7372 .4 7± 871.3）明显少于应用内腔镜切开缝合器的VATS治疗自发性气胸（12 5 2 4 .32± 2 96 2 .18元）（P <0 .0 5 ）。结论 :电视胸腔镜下肺大疱缝扎及结扎法治疗自发性气腔疗效确切

能取得与应用内腔镜切开缝合器VATS一样的治疗效果

且住院费用明显减少

该手术方法值得推广。

Abstract

Objective:To study the clinical results and the hospitalization cost of video-assisted thoracoscopic surgery (VATS) with ligation and suture for the treatment of spontaneous pneumothorax.Methods:Thirteen patients were treated by VATS with ligation and suture.The consumed time of operation

percentage of patient discontinuing anodyne within the postoperative 24 hours

the average length of hospitalization and the hospitalization cost were analysed.Results:Satisfactory therapeutic effects were found in all cases without postoperative death and complications. The average hospitalization time was 3 days.VATS with ligation and suture was preferable to transaxillary minithoracotomy (TAMT) in the clinical results.The hospitalization cost is less for VATS with ligation and suture (7372.47±871.3) than that with Endo-GIA (12524.32±2962.18) (P<0.05). Conclusion:VATS with ligation and suture is curative.The curative effect is the same in VATS with ligation and suture as in VATS.There is less hospitalization cost.
