The balance of dynamic coagulation-fibrinolysis with heart valve disease and congenital heart disease during perioperation
Objective:To observe the balance of dynamic coagulation-fibrinolysis in patients underwent heart valve replacement ( groupⅠ
n=10 ) and cases with congenital heart disease underwent surgical repair ( groupⅡ
n=10 ) with cardiopulmonary bypass. Methods:They were measured by coagulation-fibrinolysis dynamicography instrument (CFD-Ⅲ) at the pre-CPB
post-CPB
post-operative 24h
48 h and 1 week
respectively. Results:1. Post-CPB: maximum coagulation time (MCT)
the whole time of fibrinolysis reaction (WFT) of groupⅠwere significantly increased ( P<0.05 ); the balance time (BLT ) of two groups were significantly increased and longer than pre-CPB (P<0.05 ). 2. From Post-operation 24h to 1 week: maximum extent of coagulation (MCE) and content of fibrinogen (FIB) were significantly increased and continued to post-operation 1 week ; blood platelet count (PLT) remained at a lower level. 3. The graphs of modified coagulation-fibrinolysis of two groups became lower and longer at post-CPB
BLT recovered at post-operation 24h
but the peak value of graphs remained at a higher level. Conclusion The results suggest that there were more changes in the balances of two groups from post-CPB to post-operative 48h
especially in the lower level balances at post-CPB
and the changes in the cases with heart valve disease were more significant than those with congenital heart disease. The balances were at higher level from post-operative 24h to 1 week in two groups.
