Application of crosslinked sodium hyaluronate gel in the prevention of readhesion among patients after transcervical resection of adhesions
Objective:To evaluate clinical efficacy of crosslinked sodium hyaluronate gel to prevent re-adhesion for patients with moderate to severe intrauterine adhesion(IUA) after transcervical resection of adhesions. Methods:A prospective
randomized and controlled trial was designed. Fifty-two patients with moderate to severe IUA underwent transcervical resection of adhesions in Hubei Province Maternal and Children Hospital from Janunary 2016 to June 2016 were enrolled for the study and were randomly divided into two groups(observation group:26 cases
control group:26 cases). In the observation group
Foley balloon catheter and crosslinked sodium hyaluronate gel were introduced to intrauterine after operation
and the Foley balloon catheter was pulled out next day. Second-look hysteroscopic examination was performed at a week later and crosslinked sodium hyaluronate gel was given to intrauterine again at the same time. In the control group
Foley balloon catheter and common medical sodium hyaluronate gel were introduced to intrauterine after operation. Second-look hysteroscopic examination was performed and the Foley balloon catheter was pulled out at a week later
meanwhile
an intrauterine device was given to intrauterine. Artificial cycle treatment and second-look hysteroscopic examination monthly was performed to all patients. All cases were observed the shape of uterus cavity and menstruation at three months postoperatively. Results:In the observation group
the rate of recovery of uterus cavity was 80.8%(21/26)and it was 50.0%(13/26)in the control group
with statistical significant differences(P=0.02). In the observation group
the incidence of moderate to severe IUA(3.8%)was lower than that(26.9%)in the control group; The rate of menstrual improvement 84.6%(22/26)and it was 61.5%(16/26)in the control group. There was no statistical significant differences between two groups(P>0.05). The therapeutic effect in the observation group was significantly higher than that in control group(P=0.01). Conclusion:It can improve the rate of recovery of uterus cavity and therapeutic effect efficiently that crosslinked sodium hyaluronate gel as intrauterine biological barrier assists estrogen in restoration of endometrium. It is a promising way to prevent re-adhesion after transcervical resection of adhesions.
