Association between diabetic peripheral neuropathy and gait as well as balance performance in patients with type 2 diabetes mellitus
Objectives:To investigate the association of diabetic peripheral neuropathy(DPN)and biomechanics parameters such as gait and balance performance in patients with type 2 diabetes mellitus(T2DM).Methods:A cohort of 108 hospitalized T2DM patients with age-matched were admitted at southwest hospital. The subjects were divided into 3 groups based on nerve conduct velocity(NCV)and clinical symptoms or sign of DPN.The differences of biomechanics parameters were analyzed between groups
as well as the relationship with NCV.Results:(1)The hemoglobin A1c was gradually increased[(8.01±2.13)vs.(9.10±1.83)vs.(9.25±2.19)](P﹤0.05)from group A to group C.The prevalence of diabetic retinopathy was significant higher in group C than in other two groups(20.0%vs.14.7%vs.41.0%)(P﹤0.05).(2)In terms of gait parameters
the stride length/height was significant higher in group C than in other two groups[(0.49±0.14)vs.(0.52±0.10)vs.(0.44±0.11)](P﹤0.05).However
no significant difference was observed in other gait parameters.(3)In terms of static stand balance parameters
no matter eyes open or closed
the posteroanterior sway range of center of mass(Co M)in group C was significant higher than that of group A.The posteroanterior sway range of ankle angle with eyes open in group C was significant higher than that of group A[(1.10±0.41)vs.(1.26±0.52)vs.(1.46±0.62)](P﹤0.05);the sway index of ankle angle with eyes open in group C was significant higher than those of other two groups[(0.15±0.06)vs.(0.16±0.07)vs.(0.19±0.09)](P﹤0.05).The sway area of hip angle with eyes closed in group C was significant higher than those of other two groups[(1.21±0.74)vs.(1.31±0.81)vs.(2.36±0.78)](P﹤0.05).The sway index of hip angle with eyes closed in group C was significant higher than that of group A[(0.22±0.08)vs.(0.22±0.06)vs.(0.29±0.15)](P﹤0.05).(4)The sway range of Co M in all three groups with eyes closed or standing at hard surface were significant higher than that with eyes open or standing at soft surface
respectively. In modified clinical test of sensory interaction and balance(CTSIB)test
both eyes open or closed
the sway index of Co M in group C was significant higher than that of group A when stood on soft surface. Conclusion:DPN with overt clinical symptoms could decrease body balance stability under different circumstances in middle-aged and elder patients with T2DM.Ankle and hip joints compensatory strategies could improve the balance dysfunction in order to keep balance stability. Therefore
early detection with novel technology combined with preventive intervention may be a crucial measure for fall prevention.
