Individualized treatment for pediatric tibial distal epiphyseal fracture
Objective:To investigate the clinical therapeutic efficacy of individualized therapy for children with pediatric tibial distal epiphyseal fracture. Methods:Totally 196 cases of children tibial distal epiphyseal plate fracture were analyzed retrospectively in our hospital from January 2010 to December 2014. There are 134 males and 62 females aged from 1.7 to 15.0 years old with an average of(10.1±3.4)year. Based on Salter-Harris classification of fractures
31 cases were type Ⅰ
75 cases were type Ⅱ
55 cases were type Ⅲ
33 cases were type Ⅳ and 2 cases were type Ⅴ. Individualized treatment plan was formulated in view of age
injury mechanism
nutritional status
local soft tissue condition and type of fracture
etc. Results:All cases were followed up with an average of(36.5±14.6)months(10-69 months). According to Mazur ankle function assessment
the total excellent and good rate of all patients was 87.8%(172/196);the excellent and good rates for type Ⅰ
Ⅱ
Ⅲ
Ⅳ
Ⅴfractures were 100%(31/31)
88.0%(66/75)
83.6%(46/55)
84.8%(28/33)and 50.0%(1/2)
respectively. There was no significant difference in the excellent and good rate among various fractures treated by different fixation methods(P>0.05). There was no significant difference in the excellent and good rate between closed reduction and open reduction for type Ⅱ fracture below narcotism(P=1.000). Conclusion:Children with distal epiphyseal tibial fractures should be diagnosed definitely as soon as possible. The classification and individualized treatment methods have the vital clinical sig-nificance for improving the curative effect and reducing complications
