摘要

目的:观察肝脏原发性弥漫大B细胞淋巴瘤（diffuse large B cell lymphoma

DLBCL）的影像学、组织学及免疫表型

探讨其临床病理特征、诊断、鉴别诊断及治疗和预后。方法:运用组织学、免疫组化技术对5例肝脏原发性弥漫大B细胞淋巴瘤进行光镜观察及免疫标记

采用原位分子杂交方法检测淋巴瘤组织中EBV编码的小RNA（EBV-encoded small RNA

EBER）

并结合相关文献复习。结果:5例患者平均年龄63.5岁

均有右上腹部饱胀不适、消瘦伴贫血。CT增强扫描示病灶不均匀强化。组织学上

肿瘤细胞弥漫性生长

瘤细胞较大且相对一致

细胞质较丰富

可见明显核仁

核分裂象易见。免疫表型:瘤细胞弥漫表达CD19、CD20、CD79a

CD3、CD43阴性

Ki-67增殖指数75%

0%;EBER原位杂交检测3例肿瘤细胞为阳性。结论:原发性肝脏弥漫大B细胞淋巴瘤罕见

预后较差

肝活检及免疫组化标记有助于该肿瘤诊断

需注意与其他部位的淋巴瘤累及肝脏相鉴别;及早手术和化疗有望获得较好疗效。

Abstract

Objective:To investigate the radiologic

histologic

and immunohistochemical features of primary hepatic large B-cell lymphoma(PHLBL)and to explore its clinicopathologic features

diagnosis

differential diagnosis

treatment

and prognosis. Methods:Five cases of PHLBL were evaluated by histology and immunohistochemistry;the in situ hybridization was also employded to detect the EBV-encoded small RNA(EBER). The related literature was reviewed as well. Results:The 5 patients

with the average age of 63.5years

presented symptoms of upper-right abdominal distension

athrepsia

and anemia. Contrast CT scan indicated uneven enhancement in the focus. Histologically

the indistinct tumor cells showed diffused proliferation and were large in size with ample cytoplasm and overt nucleolus. Mitoses were easily observed. Immunohistochemically

the tumor cells were diffusely positive for CD19

CD20

and CD79 a

but negative for CD3 and CD43.The proliferative index of Ki67 was 75% to 80%. Conclusion:PHLBL is rare with poor prognosis. Liver biopsy and immunohistochemical stainings are helpful for its diagnosis. It should be differentiated from metastastic lymphoma involving the liver. Early surgery and chemotherapy could obtain good effect.
