Analysis of serum lipids of patients with pulmonary tuberculosis and bacterial pneumonia
Objective:To study the characteristics of serum lipids of patients with pulmonary tuberculosis and bacterial pneumonia and to explore its correlation with sputum smear results. Methods:Objects were from the Second People’s Hospital in Weifang city
Shandong province. There were there groups:460 pulmonary tuberculosis patients treated in hospital(single-tuberculosis group)
and 434 healthy check-ups in the same period(healthy controls). Objects’ fasting blood was drawn in the morning
then centrifuged and the concentration of serum total cholesterol(TC)
triglyceride(TG)
high density lipoprotein cholesterol(HDL-C)
low density lipoprotein cholesterol(LDL-C)
lipoprotein(a)(Lp(a))
apoprotein A1(Apo A1)
and apoprotein B(Apo B) was tested. TC and TG were tested by enzymic method
HDL-C and LDL-C were tested by selective-covered direct method
Lp(a)
Apo A1 and Apo B were tested by immunoturbidimetic assay. Patient ’s sputum of real time
night and next morning was selected
acid-fast staining and microscopic examination were done. The results were showed by--
1+
2+
3+
4+. Results:(1)Compared with those of healthy controls
TC
TG
HDL-C
LDLC
Apo A1 and Apo B of single-tuberculosis group were reduced(Z=-5.941
-3.733
-4.170
-6.213
-4.557
-4.022
respectively;P=0.000)
while Lp(a)was raised(Z=-2.399
P=0.016).(2)Compared with those of healthy controls
TC
TG
HDL-C
LDL-C
Apo A1 and Apo B of tuberculosis & pneumonia group were reduced(Z=-6.894
-3.676
-5.796
-6.287
-5.799
-4.321
respectively
P=0.000)
while Lp(a)was raised(Z=-2.888
P=0.004).(3)Compared with those of single-tuberculosis group
TC
HDL-C
LDL-C and Apo A1 of tuberculosis & pneumonia group were reduced(Z=-5.624
-4.169
-2.237
-4.328
respectively;0.000
0.000
0.025
0.000
respectively).(4)Sputum smear results of single-tuberculosis group were related with Apo A1(rs=-0.319
P=0.000)
while sputum smear results of tuberculosis & pneumonia group were related with HDL-C and Apo A1(rs=-0.175
-0.302
respectively;P=0.002
0.000
respectively). Conclusion:(1)TC
TG
HDL-C
LDL-C
Lp(a)
Apo A1
and Apo B are auxiliary test for the diagnosis of pulmonary tuberculosis.(2)TC
HDL-C
LDL-C and Apo A1 can be used to differentiate pulmonary tuberculosis and pulmonary tuberculosis &bacterial pneumonia.(3)Apo A1 is expected to be the disease severity standard for pulmonary tuberculosis
while HDL-C and Apo A1 are expected to be the disease severity standards for pulmonary tuberculosis & bacterial pneumonia.
