摘要

目的:系统评价急性胰腺炎患者早期进食和标准时机进食的疗效。方法:计算机检索Pubmed、Web of Science、Embase、CENTRAL、CBM、万方数据库、VIP、CNKI

查找早期进食和标准时机进食治疗急性胰腺炎的随机对照试验。检索时间截止至2015年7月。对符合纳入标准的研究进行质量评价和资料提取后采用Rev Man5.3软件进行分析。结果:共纳入5个研究

561例患者（早期进食277例vs.标准时机进食284例）

Meta分析结果显示:与标准时机进食相比

早期进食患者住院时间短（SMD=-0.56

95%CI=-0.90

0.22

P=0.001）

而进食后腹痛发生率（P=0.300）、腹胀发生率（P=0.180）、进食中断率（P=0.620）、血清淀粉酶高于正常上限率（P=0.620）无明显差异

但进食后脂肪酶高于正常上限率大于标准时机进食（P=0.040）。结论:与标准时机进食相比

急性胰腺炎患者早期进食住院时间短

进食后胃肠道并发症发生率两者无明显差异

但由于纳入研究较少

需更多大样本量研究进一步证实。

Abstract

Objective:To systematically review the efficacy and safety of early oral refeeding(EORF)and conventional oral refeeding(CORF)for acute pancreatitis. Methods:Pub Med

VIP and CNKI were searched from their inception to July

2015 for randomized controlled trials(RCTs)

comparing EORF and CORF for acute pancreatitis. The data were extracted and the methodology quality was assessed. Metaanalysis was performed using the Cochrane Collaboration’s Rev Man 5.3 software. Results:Five RCTs involving a total of 561 patients were included

277 in the experimental group and 284 in the control group. EORF was associated with shorter length of hospital stays than CORF(SMD=-0.56

95%CI=-0.90 to-0.22

P=0.001). There was no significant difference in the rate of abdominal pain relapse(P=0.300)
