Comparison of quality of life after cystectomy and different urinary diversions for bladder cancer
Objective:To evaluate quality of life of patients receiving different urinary diversions after cystectomy and to provide clinical basis for screening of urinary diversion. Methods:The clinical data of 145 patients receiving cystectomy and urinary diversion for bladder cancer from January 2011 to October 2014 were collected
including 42 cases of orthotopic urinary diversion(37 with ileal neobladder and 5 with sigmoid neobladder)
103 cases of non-orthotopic urinary diversion(84 with ileal conduit and 19 with cutaneous ureterostomy). The bladder cancer specificity scale was used to estimate the quality of life of patients. Results:Successful and effective follow-up was completed in 98 cases. Orthotopic urinary diversion group was younger than the non-orthotopic urinary diversion group(t=-3.75
P=0.000)
but the operation time and hospitalization time in orthotopic urinary diversion group was significantly increased(t=4.84
P=0.000;t=2.85
P=0.007). There were significant differences in tumor stage(χ2=0.048
P =0.827)
tumor grade(χ2=2.125
P=0.145)and histological type(P=0.660)between two groups. In urinary function field
the score of orthotopic urinary diversion group was lower than that of non-orthotopic urinary diversion group(t=-3.15
P=0.003). In the field of sexual function
two sets of scores were low
regardless of orthotopic group and non-orthotopic urinary diversion group. Older patients exhibited lower sexual function(t=2.47
P=0.019;t=4.28
P=0.000)
but no difference between the two groups was observed among the same age group(t=0.81
P=0.427;t=0.99
P=0.325). There was no difference in urinary bother(t=1.77
P =0.079)
sexual bother(t =0.32
P =0.747) and bowel function
bother about scores(t=0.30
P=0.762;t=1.12
P=0.268)between two groups. Conclusion:Non-orthotopic urinary diversion group is better than orthotopic urinary diversion group in urinary function
but is worse than orthotopic urinary diversion group in sexual function. There was no difference in urinary bother
sexual bother and bowel function between two groups.In clinical practice
emphasis should be put on health-related quality of life in postoperative patients with bladder cancer. Reasonable treatment should be chosen according to concrete situations to ensure the best quality of life.
