Tebivudine as pre-emptive treatment against hepatitis B virus reactivation in patients after chemotherapy
Objective:To investigate the anti-virus therapy effect of tebivudine on HBV reactivation in malignant tumor patients with HBV infection after their cancer chemotherapy. Methods:Totally 317 malignant tumor patients with HBV infection receiving cytotoxic chemotherapy in the Yongchuan Hospital of Chongqing Medical University were enrolled in the study. The clinical characteristics of HBV replication and liver damage in control group from January 2006 to December 2009 without tebivudine were observed. Tebivudine was used 1-2 weeks in preventive group from Januanry 2010 to December 2012 before chemotherapy
and the duration lasted for 6-12 months according to patients’ compliance after completion of chemotherapy. The clinical manifestations about HBV reactivation were investigated at the same time. Results:Of all the 317 patients after chemotherapy
96 patients(30.3%)displayed HBV reactivation. Of the 147 patients in the control group
85 patients(57.8%)displayed HBV reactivation;21 patients developed liver failure
among them
10 patients died. While
of the 170 patients in the preventive group
only 11 patients(6.5%) suffered from HBV reactivation
2 patients developed liver failure
1 patient died. There were significant differences in ratio of HBV reactivation between prevention group and control group(P<0.001). Conclusion:Low incidence of HBV reaction will appear in non-active HBs Ag carriers in the preventive group during the use of tebivudine. Patients should use tebivudine in early phase to prevent HBV reactivation and to improve clinical prognosis.
