Application of different concentrations of propofol combined with dexmedetomidine in elderly laparoscopic cholecystectomy
To analyze the anesthetic effects of different concentrations of propofol combined with dexmedetomidine in elderly patients undergoing laparoscopic cholecystectomy.
Methods
2
A total of 114 patients who underwent laparoscopic cholecystectomy were selected and divided into the group A, group B and group C according to the random number table method, with 38 cases in each group. Propofol and dexmedetomidine were used as the anesthetic drugs, and the target controlled infusion concentrations of propofol in the group A, group B and group C were 2.0 μg/mL, 3.0 μg/mL and 4.0 μg/mL respectively. The hemodynamics including heart rate, mean arterial pressure and blood oxygen saturation before anesthesia induction (T
0
), 5 minutes after tracheal intubation (T
1
), 5 minutes after pneumoperitoneum (T
2
), 15 minutes after pneumoperitoneum (T
3
) and after tracheal extubation (T
4
) were compared among groups. The excellent and good rate of anesthesia, recovery after anesthesia, intraoperative awareness, occurrence of restlessness during the awakening period and occurrence of adverse reactions during operation were compared among groups.
Results
2
There was no significant difference in blood oxygen saturation among the groups, time or interactions (
P
>
0.05). The heart rate and mean arterial pressure at T
2
and T
3
in each group were higher than those at T
0
and T
1
in each group (
P
<
0.05), and those at T
4
were lower than those at T
2
and T
3
(
P
<
0.05). Compared with T
2
, the heart rate and mean arterial pressure of T
3
in group A and group B were increased (
P
<
0.05). Compared with group A, the heart rate and mean arterial pressure at T
2
and T
3
in group B and group C were decreased(
P
<
0.05), and those in the group C were lower than those in the group B (
P
<
0.05). Compared with the group A, the excellent and good rate of anesthesia in group C was higher (
P
<
0.05). The recovery time, directional force recovery time and extubation time of group C were longer than those of group A and group B (
P
<
0.05). Both group A and B had 1 case of intraoperative awareness, while patients in group C had no intraoperative awareness, and there was no significant difference in the incidence of intraoperative awareness among groups (
P
>
0.05). The incidence of restlessness during the awakening period in group C was lower than that in the group A (
P
<
0.05). There was no significant difference in the total incidence of adverse reactions among groups (
P
>
0.05).
Conclusion
2
Different concentrations of propofol combined with dexmedetomidine have good anesthesia effect in elderly laparoscopic cholecystectomy, but the excellent and good rate of anesthesia and the incidence of restlessness during the awakening period by 4.0 μg/mL of propofolare are low, while the recovery after anesthesia by 3.0 μg/mL and 2.0 μg/mL of propofol are better than that by 4.0 μg/mL of propofol.
