Correlation study between peripheral blood NLR, hs-CRP/ALB, HMGB1 and nosocomial infection after pediatric laparoscopic intussusception reduction
To investigate the predictive value of peripheral blood neutrophil-to-lymphocyte ratio (NLR), high sensitivity C-reactive protein/albumin (hs-CRP/ALB), and high mobility group protein B1 (HMGB1) on nosocomial infection after pediatric laparoscopic intussusception reduction.
Methods
2
The clinical data of 104 children who received pediatric laparoscopic intussusception reduction in our hospital were collected, the patients were divided into the uninfected group and infection group according to the occurrence of nosocomial infection after operation, and the children in the infection group were divided into the mild group and moderate-severe group according to their infection degree. The neutrophils and lymphocytes were detected by the automatic blood cell analyzer, and NLR was calculated. The levels of hs-CRP, ALB, and HMGB1 were detected by ELISA,and hs-CRP/ALB was calculated. The
receiver operating characteristic
(
ROC
) curve was used to analyze the predictive value of peripheral blood NLR, hs-CRP/ALB and HMGB1 levels for nosocomial infection after pediatric laparoscopic intussusception reduction. The influencing factors of nosocomial infection after pediatric laparoscopic intussusception reduction were analyzed by multivariate
Logistic
regression.
Results
2
Compared with the uninfected group, the peripheral blood NLR, hs-CRP/ALB, HMGB1 levels and the proportion of children with operation time ≥ 2 hours in the infected group were higher (
P
<
0.05).The
area under the curve
(
AUC
) of NLR, hs-CRP/ALB, HMGB1 and their combination in predicting nosocomial infection after pediatric laparoscopic intussusception reduction were 0.812, 0.785, 0.724, and 0.922, respectively, with specificities of 88.6%, 85.7%, 57.1%, and 78.6%, respectively, and the sensitivities of 61.8%, 61.8%, 79.4%, and 91.2%, respectively. The increases of NLR, hs-CRP/ALB and HMGB1 were all the independent risk factors for nosocomial infection after pediatric laparoscopic intussusception reduction (
P
<
0.05).Compared with the mild group, the peripheral blood NLR, hs-CRP/ALB and HMGB1 levels of the children with intussusception in the moderate-severe group were higher (
P
<
0.05).
Conclusion
2
The abnormal increases of NLR, hs-CRP/ALB and HMGB1 levels in peripheral blood are closely related to nosocomial infection after pediatric laparoscopic intussusception reduction, which can be used as the important indicators for predicting postoperative infection and evaluating the infection degree.
