探讨AngioJet流变式机械血栓清除（ART）联合导管接触性溶栓（CDT）治疗急性下肢深静脉血栓形成的疗效及影响因素。
方法
2
回顾性分析2020年2月至2022年7月我院收治的200例急性下肢深静脉血栓形成患者的临床资料。根据手术方法将患者分为对照组和观察组，对照组100例采用CDT治疗，观察组100例采用ART联合CDT治疗。比较2组患侧与健侧肢体周径差、血栓溶解率及并发症发生率。根据观察组疗效将其进一步分为疗效差组（血栓溶解Ⅰ级，

=12）和疗效佳组（血栓溶解Ⅱ级+Ⅲ级，

=88），经二元

回归分析影响患者治疗效果的独立因素。

观察组治疗后3个月患侧与健侧大腿周径差、小腿周径差均小于对照组（

ART联合CDT治疗急性下肢深静脉血栓形成可提高溶栓成功率，但疗效仍受治疗时间、血栓时间、年龄影响，对此需针对高危因素进行相关干预，以提高整体疗效。
Abstract
Objective

To evaluate the efficacy and influencing factors of AngioJet rheolytic thrombectomy （ART） combined with catheter-directed thrombolysis （CDT） in the treatment of acute lower extremity deep venous thrombosis.
Methods

The clinical data of 200 patients with acute lower extremity deep venous thrombosis admitted to our hospital from February 2020 to July 2022 was analyzed retrospectively.According to the surgical methods，the patients were divided into the control group and the observation group，100 patients in the control group were treated with CDT，and 100 patients in the observation group were treated with ART combined with CDT.The limb circumference difference between affected side and healthy side，thrombolysis rate and complication rate were compared between the two groups.According to the efficacy of patients in the observation group， the patients was further divided into the poor efficacy group （thrombolysis grade Ⅰ，
n
=12） and the good efficacy group （thrombolysis grade Ⅱ+Ⅲ，
n
=88）.The independent factors affecting the therapeutic effect of patients were analyzed by binary
Logistic
regression.
Results

The differences of thigh circumference and calf circumference between affected side and healthy side in the observation group were less than those in the control group （
P
<
0.05）， and the thrombolysis rate was higher than that in the control group （
P
<
0.05）. There was no significant difference in the incidence of postoperative complications between the two groups （

0.05）. According to the binary

regression analysis， the age ≥ 60 years old， non-first treatment and thrombosis time ≥ 7 hours were the independent factors affecting the treatment effect of patients （

Conclusion

ART combined with CDT for acute deep venous thrombosis of lower extremity can improve the success rate of thrombolysis，but the efficacy is still affected by the treatment time，thrombosis time and age，therefore relevant intervention for high risk factors should be carried out in order to improve the overall efficacy.
