摘要

目的

对比分析机器人直肠癌手术吻合口不同加固缝合方式的临床疗效。

方法

选取我院收治的127例机器人直肠癌手术患者作为研究对象，按照随机数字表法分为未加固组（42例）、全层加固缝合组（42例）、浆肌层加固缝合组（43例）。比较各组患者的围术期相关指标、术后并发症发生情况。

结果

与未加固组相比，全层加固缝合组和浆肌层加固缝合组的手术时间增加，术中出血量、术后胃肠道排气时间、恢复进食时间、住院费用、住院时间均减少/缩短（

0.05）；浆肌层加固缝合组的各项指标均优于全层加固缝合组（

0.05）。各组患者术后并发症发生率比较差异有统计学意义（

=0.031），其中未加固组术后并发症发生率最高（

0.05），浆肌层加固缝合组最低（

0.05）。

结论

在机器人直肠癌手术中对吻合口进行加固缝合有利于患者的早期康复，能够降低术后并发症发生率，且浆肌层加固缝合疗效优于全层加固缝合。

Abstract

Objective

To compare and analyze the clinical effects of different suture reinforcements on the anastomotic stoma of robotic rectal cancer surgery.

Methods

A total of 127 patients with robotic rectal cancer surgery in our hospital were selected as the research subjects and divided into the unreinforced group （42 cases），all layers reinforced suture group （42 cases） and seromuscular layer reinforced suture group（43 cases） according to the random number table method.The perioperative indicators and postoperative complications were compared among the three groups.

Results

Compared with the unreinforced group，the operative time in the all layers reinforced suture group and the seromuscular layer reinforced suture group were increased，and the amount of intraoperative blood loss，postoperative gastrointestinal exhaust time，time to resume feeding，hospitalization cost and length of hospital stay were decreased/shortened （

0.05）.The indexes of the seromuscular layer suture group were better than those of the all layers reinfored suture group （

0.05）.There were significant differences in the incidence of postoperative complications among all groups （

=0.031）.The highest incidence of postoperative complications was in the unreinforced group（

0.05） and the lowest was in the seromuscular layer suture group（

0.05）.

Conclusion

Suture reinforcement of anastomosis in robotic rectal cancer surgery is beneficial to the early rehabilitation of patients，which can reduce the incidence of postoperative complications，and the effect of seromuscular layer reinforced suture is better than that of all layers reinforced suture.
