摘要

目的

探讨酒石酸布托啡诺对冠状动脉旁路移植术（CABG）患者丙泊酚麻醉诱导期血流动力学的影响。

方法

将纳入的94例行CABG治疗的患者随机分为观察组和对照组，各47例。观察组患者麻醉诱导前给予40 μg/kg酒石酸布托啡诺，对照组患者麻醉诱导前给予等量生理盐水，2组麻醉诱导及维持方法相同。于给予酒石酸布托啡诺或生理盐水前（T0）、插管即刻（T1）、插管后1 min（T2）、插管后3 min（T3）、插管后5 min（T4）记录2组患者平均动脉压（MAP）、心率（HR），同时检测血清皮质醇（Cor）、促肾上腺皮质激素（ACTH）、去甲肾上腺素（NE）水平。统计2组患者麻醉诱导期不良反应发生情况。

结果

2组患者T1时MAP、HR均明显低于T0时（

0.05）；对照组T2~T4时MAP、HR较T0时明显升高（

0.05），观察组T2~T4时MAP、HR较T0时明显降低（

0.05）；T1时观察组MAP、HR明显较对照组高（

0.05），T2~T4时观察组MAP、HR明显较对照组低（

0.05）。观察组T2时血清Cor、ACTH、NE水平较T0时明显升高（

0.05）；对照组T1~T4时血清Cor、ACTH、NE水平较T0时升高（

0.05），观察组T1~T4时血清Cor、ACTH、NE水平均明显较对照组低（

0.05）。观察组麻醉诱导期低血压、高血压发生率均较对照组低（

0.05）。

结论

酒石酸布托啡诺用于CABG中有利于减轻患者丙泊酚麻醉诱导期的应激反应，避免患者丙泊酚麻醉诱导期血流动力学的剧烈波动。

Abstract

Objective

To investigate the effect of butorphanol tartrate on hemodynamics during anesthesia induction with propofol in patients undergoing coronary artery bypass grafting （CABG）.

Methods

A total of 94 patients received CABG treatment were randomly divided into the observation group and the control group，with 47 cases in each group.Patients in the observation group were given 40 μg/kg butorphanol tartrate before anesthesia induction，patients in the control group were given the same amount of normal saline before anesthesia induction.The anesthesia induction and maintenance of the two groups were given in the same way.The mean arterial pressure （MAP） and heart rate （HR） were recorded before administration of butorphanol tartrate or normal saline（T0），immediately after intubation （T1），1 minute after intubation （T2），3 minutes after intubation （T3） and 5 minutes after intubation （T4），and the serum levels of cortisol （Cor），adrenocorticotropic hormone （ACTH） and norepinephrine （NE） at the same time points in the two groups were detected.The incidences of adverse reactions during anesthesia induction in the two groups were statistically analyzed.

Results

MAP and HR at T1 in the two groups were significantly lower than those at T0 （

0.05），MAP and HR from T2 to T4 in the control group were significantly higher than those at T0 （

0.05），MAP and HR from T2 to T4 in the observation group were significantly lower than those at T0 （

0.05）. MAP and HR at T1 in the observation group were significantly higher than those in the control group （

0.05）， MAP and HR from T2 to T4 in the observation group were significantly lower than those in the control group （

0.05）.The serum levels of Cor，ACTH and NE at T2 in the observation group were significantly higher than those at T0 （

0.05），the serum levels of Cor，ACTH and NE from T1 to T4 in the control group were significantly higher than those at T0 （

0.05），the serum levels of Cor，ACTH and NE from T1 to T4 in the observation group were significantly lower than those in the control group （

0.05）.The incidences of hypotension and hypertension during anesthesia induction in the observation group were significantly lower than those in the control group （

0.05）.

Conclusion

The application of butorphanol tartrate in CABG is beneficial to alleviate the stress response of patients and avoid the drastic fluctuation of hemodynamics during anesthesia induction with propofol.
