Clinical effects of internal fixation via different approaches for treatment of femoral shaft fractures
To compare the effects of internal fixation via different approaches in the treatment of femoral shaft fractures.
Methods
2
The clinical data of 88 patients with femoral shaft fractures treated by unilateral approach surgery in our hospital were analyzed retrospectively. According to the different surgical approaches, the patients were divided into the lateral approach group (
n
=40) and the posterolateral approach group (
n
=48), and treated with internal fixation via lateral femoral approach and internal fixation via posterolateral femoral approach, respectively. The operation time, intraoperative blood loss, fracture healing time, treatment cost of the two groups were recorded. The pain degree and soft tissue swelling in the middle thigh before surgery, 1 week and 3 months after surgery of the two groups were evaluated. The knee joint function within 1 year after surgery and the incidence of complications of the two groups were compared.
Results
2
Compared with the lateral approach group, the posterolateral approach group had less intraoperative blood loss, shorter fracture healing time, lower hospitalization and total costs, with statistically significant differences (
P
<
0.05). There was no significant difference in operation time or cost between the two groups (
P
>
0.05). The postoperative visual analogue scale (VAS) score and differences of soft tissue swelling in the middle thigh between the two groups were statistically significant (
P
<
0.05). The excellent and good rate of knee joint function in the posterolateral approach group was 91.67%, which was higher than 72.50% in the lateral approach group, and the difference was statistically significant (
P
<
0.05). There was no significant difference in the incidence of complications between the two groups (
P
>
0.05).
Conclusion
2
Internal fixation via posterolateral femoral approach for femoral shaft fractures can reduce intraoperative blood loss, shorten fracture healing time, relieve postoperative pain and tissue edema of patients, restore knee joint function, and the treatment cost is relatively low, which is safe and reliable.
