摘要

目的

探讨关节镜联合胫骨高位截骨术治疗膝内侧骨关节炎合并髌股关节炎的临床疗效。

方法

纳入我院采用关节镜结合胫骨高位截骨术治疗的膝内侧间室合并髌股关节炎患者15例，患者先行关节镜下髌骨成形、外侧支持带松解、髌周去神经化手术，再行胫骨高位截骨术。记录患者手术相关情况，比较患者术前及术后6个月膝关节美国特种外科医院(HSS)评分，疼痛视觉模拟量表(VAS)评分，测量手术前后胫股角(FTA)评估临床疗效。

结果

患者手术均顺利完成，手术时间（90±26）min，术后切口均Ⅰ期愈合，术中及术后均无严重并发症发生。相较术前，患者术后膝关节疼痛症状均有所改善，术后12周均骨性愈合。术后FTA矫正了9°~25°，纠正至正常范围内，临床疗效显著，与术前比较显著改善（

0.05）。与术前对比，术后6个月VAS评分显著降低，膝关节HSS评分明显升高，差异均有统计学意义（

0.05）。

结论

关节镜联合胫骨高位截骨术治疗膝内侧骨关节炎合并髌股关节炎的近期疗效较好，但远期疗效有待进一步观察。

Abstract

Objective

To explore the clinical efficacy of arthroscopy combined with high tibial osteotomy in treatment of knee medial compartmental osteoarthritis and patellofemoral arthritis.

Methods

A total of 15 patients with knee medial compartmental osteoarthritis and patellofemoral arthritis who treated by arthroscopy combined with high tibial osteotomy in our hospital were included， they all received arthroscopic patelloplasty，lateral patellar retinaculum release and circumpatellar denervation， and then high tibial osteotomy.The surgery-related conditions of patients were recorded， and the American hospital for special surgery （HSS） score of the knee joint and the pain visual analogue scale （VAS） score before and 6 months after surgery were compared.The femorotibial angles （FTA） before and after surgery were measured to evaluate the clinical efficacy.

Results

The surgeries of patient were successfully completed， with an average operation time of （90±26） minutes.The postoperative incisions healed in stage I，without serious complications during or after surgery.The symptoms of postoperative knee pain were improved compared with those before surgery，with bone healing of 12 weeks after surgery.Postoperative FTA was corrected by 9°~25°to the normal range， and the clinical efficacy was remarkable， which was significantly improved compared with that before surgery （

0.05）.The VAS score 6 months after operation decreased significantly and the HSS score of knee joint 6 months after operation increased significantly compared with those before surgery， with statistically significant differences （

0.05）.

Conclusion

Arthroscopy combined with high tibial osteotomy in the treatment of knee medial compartmental and patellofemoral arthritis has good short-term clinical efficacy， but the long-term efficacy needs further observaiton.
