摘要

目的:探讨腭咽肌切断术治疗阻塞性睡眠呼吸暂停低通气综合征（Obstructive sleep apnea hypopnea syndrome

OSAHS）患者术后一年阻塞性呼吸暂停指数（Obstruetive apaea index

OAI）、中枢性呼吸暂停指数（Central apaea index

CAI）、混合性呼吸暂停指数（Midex apaea index

MAI）和低通气指数（Hypopaea index

HI）的变化及各型呼吸紊乱的相关性。方法:经夜间7h多导睡眠监测诊断为OAI患者49例

均行低温等离子射频消融腭咽肌切断术治疗

以术前和术后1年的多导睡眠监测及主观感觉等指标作对照。结果:术后1年复查:43例患者睡眠打鼾

晨起嗜睡等症状有改善

41例OAI降低

38例CAI指数降低

40例MAI降低

36例低通气指数降低。阻塞性呼吸暂停减少可以减少中枢性呼吸暂停

低通气次数减少可以减少中枢性呼吸暂停。结论:OAI、CAI和HI可相互影响;低温等离子射频腭咽肌切断术创伤小

有效地改善OSAHS患者的缺氧症状。

Abstract

Objective:To explore the effect of dissection of palatopharyneus muscle and the relation amang obstructive apnea index(OAI)

central apnea index(CAI)

mixed apnea index(MAI)and hypopnea index(HI)in patients with obstructive sleep apnea hypopnea syndrome(OSAHS) after operation.Methods:49 patients were diagnosed with OSAHS by Polysomnography(PSG) before operation.Dissection of palatopharyneus muscle was undertaken in these patients.All patients were followed one year after operation.Results:43 patients improved with reduced snoring and daytime sleepiness one year after opreation.OAI in 41 patients decreased

CAI in 38 patients decreased

MAI in 40 patients decreased

and HI in 36 patients decreased.CAI decresed with OAI and HI.Conclusion:Dissection of palatopharyneus muscle is effective to alleviate the hypoxia in patients with OSAHS.
