Clinical report of lung recruitment maneuver combined with low tidal volume in the treatment of twenty-two cases of neonatal acute lung injury
Objective:To explore the effect and feasibility of recruitment maneuver(RM) combined with low tidal volume in neonatal acute lung injury. Methods:A prospective physiologic study was conducted in the neonatal intensive care unit(NICU).Twenty-two consecutive eligible neonatal patients with ALI according to the 1994 AECC definition were enrolled in this study. The children in RM group received the ventilation of RM with low tidal volume. The clinician administered RM was performed at 10 cm H2O continuous positive airway pressure(CPAP) for 30 seconds.RMs were conducted once every eight hours for three days.Data on gas exchange
lung mechanics and hemodynamics from pre-RM to post-RM(at 12
24
48 and 72 hours) were recorded. To monitor the pathogenic conditions of the children
the radiographic examination was repeated within 3-5 days after administration. Results:The values of PaO2
PaO2/FiO2 and Cst in RM group significantly increased compared with baseline data and the values in non-RM group at 12
24
48 and 72 hours after RM was administrated(P<0.05).However
the values of PaCO2 and PIP markedly decreased compared baseline and non-RM group at the same time point(P<0.05).No significant difference of heart rate and blood pressure was found between before and after RM was administered in RM group(P>0.05).Conclusion:RM can significantly improve the oxygenation and attenuate the deterioration of pulmonary function in the treatment for neonatal ALI
and can safely and feasibly improve the lung compliance.
