Effect of early psychological intervention on successful weaning of ventilator-dependent patients
Objective To investigate the effect of early psychological intervention on successful weaning in ventilator-dependent patients. Methods A total of 120 ventilator-dependent patients admitted to a hospital in Henan Province from October 2018 to December 2020 were selected as the study subjects
who were randomly assigned to the observation group(n=60) and the control group(n=60). The patients in the control group received conventional weaning therapy and respiratory rehabilitation training
and the patients in the observation group early psychological nursing on top of the treatments in the control group. After 2 weeks of intervention
the successful weaning rate and successful weaning time were compared between the 2 groups. Heart rate
respiratory rate
partial pressure of oxygen(PaO
2
) and partial pressure of carbon dioxide(PaCO
2
) were compared between the two groups 24 h before the intervention and 2 h after the intervention. The self-rating anxiety scale(SAS) and self-rating depression scale(SDS) scores were compared between the two groups. Results The success rate of weaning in the observation group was 91.67%
which was significantly higher than that in the control group(71.67%)
with statistical significance(P
<
0.05). The weaning time of patients in the observation group was(8.43±3.21) d
which was shorter than that in the control group(12.15±4.35) d
and the difference was statistically significant(P
<
0.05). Before the intervention
there was no significant difference in heart rate
respiratory rate
PaCO
2
and PaO
2
levels between the 2 groups(P
>
0.05). After the intervention
heart rate
respiratory rate and PaCO
2
level in the two groups were lower than those before the intervention
and the observation group was lower than the control group
with statistical significance(P
<
0.05). After the intervention
PaO
2
in the two groups was higher than that before the intervention
and the observation group was higher than the control group
with statistical significance(P
<
0.05). Before the intervention
there was no significant difference in SAS and SDS scores between the two groups(P
>
0.05). After the intervention
SAS and SDS scores in the two groups were lower than those before the intervention
and the observation group was lower than the control group
with statistical significance(P
<
0.05). Conclusion Early psychological intervention in combination with respiratory rehabilitation training can significantly increase the success rate of ventilator weaning in patients with ventilator dependence
shorten the weaning time
and improve the respiratory
blood gas indicators and psychological status of patients. These benefits justify a wider application of this procedure in clinical practice.
