Influence of autologous blood transfusion technology on coagulation function, inflammatory factors and immune function in patients undergoing hip replacement under general anesthesia
Objective To investigate the effect of autologous blood transfusion technology on coagulation function
inflammatory factors and immune function in patients undergoing hip replacement under general anesthesia. Methods A total of 96 patients who underwent hip replacement under general anesthesia in a hospital in Mianyang City from February 2018 to January 2020 were selected as subjects. They were assigned to the control group(n=48) and the observation group(n=48) by the random number table method. The control group received routine perioperative management intervention
while the observation group received autologous blood transfusion technology intervention. The general operating conditions of the patients in the two groups were compared. The coagulation functions [prothrombin time(PT)
fibrinogen(FIB)
activated partial thrombostin time(APTT)
inflammatory factors [interleukin-6(IL-6)
interleukin-10(IL-10) and tumor necrosis factor-α(TNF-α)] and immune function indicators [immunoglobulin A(IgA)
immune globulin G(IgG) and immunoglobulin M(IgM)] were compared. Results There was no significant difference in operation time
intraoperative blood loss and intraoperative fluid replacement volume between the two groups(P>0.05). The amount of allogeneic blood transfusion in the observation group was lower than that in the control group
and the difference was statistically significant(P<0.05). The average volume of autologous blood transfusion in the observation group was 281.04±53.17 mL. At 7 d post-operation
the PT and APTT of the two groups were shorter than those at 1 d pre-operation
and the observation group was lower than the control group
with statistical significance(P<0.05). At 7 d post-operation
the FIB of both groups was higher than that at 1 d pre-operation
and the observation group was higher than the control group
with statistical significance(P<0.05). At 7 d post-operation
the levels of IL-6 and TNF-α in both groups were lower than those at 1 d pre-operation
and the observation group was lower than the control group
with statistical significance(P<0.05). At 7 d post-operation
the levels of IL-10 in the two groups were higher than those at 1 d pre-operation
and the observation group was higher than the control group
with statistical significance(P<0.05). At 7 d post-operation
the levels of IgA
IgM and IgG in the two groups were lower than those at 1 d pre-operation
and the differences were statistically significant(P<0.05). At 7th d post-operation
the levels of IgA
IgM and IgG in the observation group were higher than those in the control group
and the differences were statistically significant(P<0.05). Conclusion Autologous blood transfusion technology can effectively reduce the amount of allogeneic blood transfusion in patients undergoing hip replacement under general anesthesia
improve the coagulation function and immune function of patients
and reduce the inflammatory response. These benefits justify a wider application of this technique in clinical practice.
