Study on the efficacy for the treatment of moderate-to-severe endometriosis combined with infertility of laparoscopic debridement combined with leuprolide and progesterone respectively
Objective To investigate the clinical efficacy of leuprolide and progesterone respectively combined with laparoscopic debridement for the treatment of moderate-to-severe endometriosis combined with infertility. Methods One hundred and eighteen patients with moderate-to-severe endometriosis combined with infertility admitted to a hospital in Nanyang from January 2015 to February 2019 were selected for the study. The patients were divided into the leuprolide combination group(59 cases) and proge sterone combination group(59 cases) using a random number table.Patients in the leuprolide combination group were treated with leuprolide combined with laparoscopic debridement
and patients in the proge sterone combination group were treated with proge sterone combined with laparoscopic debridement. The subjective symptom scores and laboratory indexes [serum follicle stimulating hormone(FSH)
luteinizing hormone(LH)
matrix metalloproteinase-9(MMP-9)
tissue inhibitory factor of metalloproteinase-1(TIMP-1)] before and after treatment were compared between the 2 groups;the treatment effects and the recurrence and pregnancy rates within 6 months after treatment were compared between the 2 groups. Results The total treatment efficacy of patients in the leuprolide combination group was 94.92%
which was higher than that of patients in the proge sterone combination group(93.22%)
but the difference was not statistically significant(P>0.05).Before treatment
the differences in subjective symptom scores
serum FSH
LH
MMP-9 and TIMP-1 levels were not statistically significant(P>0.05);after treatment
the subjective symptom scores
serum FSH
LH
MMP-9 and TIMP-1 level-s were lower in both groups than before treatment
and those in the leuprolide combination group was significantly lower than the proge sterone combination group(P<0.05).At 6-month follow-up after treatment
one case of detachment occurred in both the leuprolide combination group and the proge sterone combination group
and the recurrence rate was 5.17% in the leuprolide combination group
which was lower than that in the proge sterone combination group(20.69%)
with a statistically significant difference(P<0.05);the pregnancy rate was 60.34% in the leuprolide combination groupwhich was higher than that in the progesterone combination group(55.17 %)
but the difference was not statistically significant(P> 0.05).Conclusion The combination of leuprolide and laparoscopic debridement for the treatment of moderate-to-severe endometriosis combined with infertility is effective and can effectively improve the subjective symptoms of patients
protect ovarian function and prevent the recurrence of the disease.
