Short-term efficacy of arthroscopic surgery combined with early rehabilitation training in the treatment of recurrent anterior shoulder dislocation associated with Hill-Sachs lesions
Objective:To explore the short-term efficacy of arthroscopic Bankart repair and remplissage combined with early rehabilitation training for recurrent anterior shoulder dislocation associated with Hill-Sachs lesions.Methods:Twenty-five patients with recurrent anterior shoulder dislocation admitted to our hospital from January2016 to June 2019 were enrolled as subjects. In those patients
19 were male and 6 female
with a mean age of 29.6 ±10.0 years(18 to 47 years). Those patients received arthroscopic Bankart repair and remplissage combined with early rehabilitation training
and > 6 months of follow-up was performed after surgery. We measured the range of shoulder motion and assessed shoulder function by Rowe
Oxford Shoulder Instability(OSIS)
and Constant-Murley scores before surgery and at the last follow-up. The efficacy of the combination therapy was analyzed. Results:All patients underwent successful surgery and had satisfactory wound healing and no obvious complications. The mean follow-up was 18 ± 9.4(6 ~ 36) months. No re-dislocation was observed at the last follow-up. The result of apprehension test was negative. The range of shoulder motion was relatively normal. There were no significant reductions in the ranges of external rotation
internal rotation
flexion
or abduction at 0° position after surgery(57.2 ± 8.2° vs 52.8 ± 7.3°
9.3 ± 3.1° vs 8.5 ± 2.1°
163.0 ± 6.4° vs 159.6 ± 6.3°
168.9 ± 9.5° vs 167.2 ± 7.8°
all P > 0.05). The Rowe
OSIS
and Constant-Murley scores for shoulder joint were all significantly improved after surgery(36.2 ± 9.8 vs 89.6 ±6.5
36.6 ± 7.1 vs 17.6 ± 3.6
59.4 ± 8.9 vs 90.4 ± 7.3
all P < 0.01). Conclusion:In the treatment of recurrent anterior shoulder dislocation associated with Hill-Sachs lesions
arthroscopic surgery combined with early rehabilitation training effectively restores shoulder stability and improves shoulder function with minimal invasion
rapid recovery
and no substantial impact on the range of shoulder motion.
