Efficacy of metformin hydrochloride combined with insulin aspart for gestational diabetes and its effect on maternal and neonatal outcomes
Objective To assess the efficacy of metformin hydrochloride combined with insulin aspart in treating gestational diabetes and its effect on maternal and neonatal outcomes. Methods A total of 80 patients with gestational diabetes treated at a Henan hospital between January 2016 and September 2017 were included in this study.Based on a random number table
patients were randomized into the control group(routine care+insulin aspart)or the trial group(routine care + insulin aspart+ metformin hydrochloride). The preand post-treatment fasting blood glucose(FBG)
2 hpostprandial glucose(2 hPG)
glycated hemoglobin(HbA1 c)
total cholesterol(TC)
total bilirubin(TBil)
uric acid(UA)
micro-albunminuria(mAlb)as well as maternal and neonatal outcomes were compared between the two groups. Results The pre-treatment FBG
2 hPG and HbA1 clevels were similar between the two groups(P>0.05).After treatment
the FBG
2 hPG and HbA1 clevels were decreased in both groups when compared with their pre-treatment levels
but significantly greater reductions were observed in the trial group compared with the control group(P<0.05).The pre-treatment TC and TBil levels were comparable between the two groups(P>0.05).After treatment
the TC level was decreased and the TBil level was increased in both groups when compared with their pre-treatment levels;the TC level in the trial group was significantly lower than that in the control group
while the TBil level was significantly higher than that in the control group(P<0.05).The pre-treatment UA and mAlb levels were similar between the two groups(P>0.05).After treatment
the UA and mAlb levels were decreased in both groups when compared with their pre-treatment levels
but significantly greater reductions were noted in the trial group compared with the control group(P<0.05).The incidences of hypertension
hydramnios
premature delivery
and cesarean delivery in the trial group were significantly lower than those in the control group(P<0.05)
and the risks for neonatal macrosomia
jaundice
respiratory distress were also significantly lower in the trial group compared with the control group(P<0.05).Conclusion For women with gestational diabetes
metformin hydrochloride combined with insulin aspart is safe and effective in decreasing the blood glucose
