Effect of different doses of bisoprolol on heart rate of patients with congestive heart failure
Objective To assess the effect of different doses of bisoprolol on the heart rate of patients diagnosed with congestive heart failure(CHF).Methods A total of 173 patients with CHF treated at a Chongqing hospital between April 2015 and June 2016 were included in this study.Using a random number table
these patients were divided into the control group(n=42)
low-dose group(n=45)
medium-dose group(n=42)
and high-dose group(n=44).Patients in the control group received routine treatment with diuretic+angiotensin-converting enzyme inhibitor(ACEI)+vasodilator+digoxin;while those in the low-
medium-
and highdose groups received routine treatment plus bisoprolol. The resting causal heart rate
maximal heart rate
minimal heart rate
mean heart rate
daytime mean heart rate(8:00-20:00)
and nighttime mean heart rate(20:00-8:00 of the following day)were measured and compared before treatment and at 6 months after treatment initiation.Besides
the P-R interval
QRS duration
and QT interval were also compared at 6 months. Results At 6 months
the resting causal heart rate
maximal heart rate
minimal heart rate
mean heart rate
and daytime mean heart rate were comparable between the control group and the low-dose group(P>0.05).In the medium-and high-dose groups
the resting causal heart rate
maximal heart rate
minimal heart rate
mean heart rate
and daytime mean heart rate were significantly lower than those in the control group(P<0.05)
and the resting causal heart rate
maximal heart rate
mean heart rate
and daytime mean heart rate were significantly lower than those in the low-dose group(P<0.05).The nighttime mean heart rate was similar among the 4 groups(P>0.05).The resting causal heart rate
maximal heart rate
minimal heart rate
mean heart rate
and daytime mean heart rate were comparable between the medium-and high-dose groups(P>0.05).No significant differences were noted in the P-R interval
QRS duration
and QT interval among the 4 groups(P>0.05).Conclusion In patients with CHF
bisoprolol at a medium-or high-dose is more effective than low-dose without any increased risk for atrioventricular block.Based on these findings
medium-or high-dose bisoprolol is safe and effective in treating CHF.
