摘要

目的

探讨颅脑创伤后下肢深静脉血栓(deep venous thrombosis

DVT)发生的危险因素。

方法

以本科2007年10月1日至2017年10月1日的颅脑创伤患者为研究对象，采用病例对照研究方法，筛选颅脑创伤合并DVT的患者112例为病例组，以1 :2的比例选取同期入院的颅脑创伤但非下肢DVT患者224例为对照组，收集患者的一般人口学资料、实验室检查和药物治疗等情况并进行比较，分析下肢DVT发生的危险因素。

结果

病例组与对照组性别、冠心病、高脂血症、高血压、糖尿病、出院转归、激素药、WBC、Alb和PLT等比较，差异无统计学意义(

0.05)；两组年龄、平均住院时间、BMI＞30 kg/m

、肺部感染、脑梗死、抗凝类药物、脱水类药物、溶栓类药物和D-二聚体等比较，差异有统计学意义(

0.05)。多因素条件Logistic回归分析显示：高龄、D-二聚体升高、脱水药、长住院时间和肺部感染是下肢DVT发生的独立危险因素(

0.05)。

结论

颅脑创伤后下肢DVT可能与高龄、D-二聚体升高、脱水药、长住院时间和肺部感染等有关。

Abstract

Objective

To explore the risk factors of lower limb deep venous thrombosis (DVT) after traumatic brain injury (TBI).

Methods

This hospital-based case-control study was conducted among 112 patients diagnosed with TBI and DVT between 2007 and 2017 in our department

with another 224 patients with TBI but without DVT serving as the control group (with a case: control ratio of 1 :2). The demographic characteristics

laboratory findings and medications of the patients were compared between the 2 groups.

Results

The gender distribution

comorbidity rates (coronary artery disease

hyperlipemia

hypertension

and diabetes)

treatment outcomes

use of steroids

white blood cell count

number of patients with albumin level

30 g/L

and platelet levels showed no significant differences between the 2 groups (

0.05). Significant differences were noted between the groups in age

average hospital stay

percentage of patients with a body mass index (BMI)

30 kg/m

incidences of lung infections and stroke

and use of anticoagulants

dehydrants

and thrombolytics

and D-dimer level (

0.05). Multivariate conditional logistic regression analysis identified an older age

a higher D-dimer level

use of dehydrants

a longer hospital stay and lung infections as the independent risk factors of DVT in patients with TBI (

0.05).

Conclusion

The occurrence of lower limb DVT after TBI is associated with an older age

a high D-dimer level

the use of dehydrants

prolonged hospital stay and lung infections.
