Objective
2
To determine the effect of small incisions combined with negative pressure wound therapy on the healing of limb swelling and inflammatory response induced by snakebite.
Methods
2
Twenty-nine patients diagnosed as snakebite admitted in our emergency department from April 2015 to July 2016 were enrolled in this study. They were 18 males and 11 females
at an age of 18~65. The patients were randomly divided into control group and observation group. The patients of control group received the conventional comprehensive treatment. And those of the observation group were given with the negative pressure suction after a surgery in proximal limb skin by multiple small incisions
and the conventional comprehensive treatment at the same time. At the initial time of treatment
and in 6
12
24
48 and 72 h during treatment
the perimeters of injured limb on the proximal
middle and distal sites
and the corresponding sites of the unaffected limb were measured and recorded to calculate the difference values. At the time points
the plasma contents of IL-6 and TNF-α were detected. The above results
and the length of hospital stay were compared between the 2 groups. This study was registered and approved by the Chinese Clinical Trail Registration and Ethnic Review Commission (registration number: ChiCTR-TRC-14004189).
Results
2
Significance differences were found in the differences of the perimeters of injured limb to the unaffected limb between the observation group to the control group
with the former obviously lower than the latter group at all the time points (
P
<
0.05). The plasma contents of IL-6 and TNF-α were remarkably lower in the observation group than the control group at all time points after treatment (
P
<
0.05). Significant difference was also found in the average length of hospital stay between the 2 groups
with the observation group shorter (5.43±1.07
vs
7.67±1.95 d
P
=0.001).
Conclusion
2
Small incisions combined with negative pressure wound therapy on the basis of conventional comprehensive treatment can effectively control the limb swelling
reduce the systemic inflammatory responses
and shorten the length of hospital stay.
