摘要

目的

探讨中老年女性中，焦虑和抑郁症状在膳食营养与围绝经期症状之间的多重中介效应。

方法

选取1 129名中老年女性为调查对象，使用膳食营养状况问卷、改良Kupperman指数（modified Kupperman index，MKI）、患者健康问卷抑郁量表（the patient health questionnaire，PHQ-9）和焦虑障碍量表（the generalized anxiety disorder，GAD-7）进行问卷调查，采用SPSS PROCESS宏进行多重中介效应分析。

结果

膳食营养、焦虑、抑郁症状和围绝经期症状之间均明显相关（

0.01）。膳食营养对围绝经期症状有明显直接影响（

=0.06，95%CI=0.002~0.119），同时还可以通过以下3条路径产生间接的负面影响：①焦虑（

=0.059，95%CI=0.031~0.091），占总效应的29.65%；②抑郁（

=0.034，95%CI=0.017~0.054），占总效应的17.29%；③焦虑-抑郁（

=0.045，95%CI=0.024~0.073），占总效应的22.76%。

结论

膳食营养不仅对围绝经期症状有直接影响，还通过焦虑和抑郁的中介作用影响围绝经期症状的发生。

Abstract

Objective

To investigate the multiple mediating effects of anxiety and depression on the relationship between dietary nutrition and perimenopausal symptoms in middle-aged and elderly women.

Methods

A total of 1129 middle-aged and older women were surveyed using a dietary nutrition status questionnaire，the modified Kupperman Index，the Patient Health Questionnaire-9，and the Generalized Anxiety Disorder 7-item scale. The multiple mediating effect analysis was performed using the PROCESS macro in SPSS.

Results

Dietary nutrition，anxiety，and depression were all significantly associated with perimenopausal symptoms（

0.01）. Dietary nutrition had a significant direct impact on perimenopausal symptoms（

=0.06，95%CI=0.002-0.119），and also exerted indirect negative effects through three pathways：anxiety（

=0.059，95%CI=0.031-0.091），accounting for 29.65% of the total effect； depression（

=0.034，95%CI=0.017-0.054），accounting for 17.29% of the total effect； and anxiety-depression（

=0.045，95%CI=0.024-0.073），accounting for 22.76% of the total effect.

Conclusion

Dietary nutrition not only directly affects perimenopausal symptoms，but also influences them through the mediating effects of anxiety and depression.
