摘要

目的

探讨全飞秒激光手术后不同近视程度患者的眼表情况及视觉质量。

方法

选取2022年1月至2024年12月于我院行全飞秒激光手术的近视患者208例，根据近视程度分为高度近视组102例（204眼）及中低度近视组106例（212眼）。比较2组患者手术前后裸眼视力（UCVA）、最佳矫正视力（BCVA）、眼表情况、视觉质量及术后并发症。

结果

高度近视组患者术后1周、2周、1个月、3个月、6个月UCVA及BCVA对应的LogMAR值均大于中低度近视组（

0.05）。高度近视组患者术后1周、2周、1个月、3个月患眼泪河高度均大于中低度近视组（

0.05），脂质层厚度分级评分均高于中低度近视组（

0.05）。高度近视组患者术后1周、2周主观视觉质量问卷评分均低于中低度近视组（

0.05）。2组患者术后6个月内均未出现视网膜脱离、青光眼、圆锥角膜、角膜浑浊、感染等并发症。

结论

对于不同近视程度的患者，全飞秒激光手术治疗均有良好效果及安全性，而中低度近视患者术后早期眼表情况及视觉质量优于高度近视患者。

Abstract

Objective

To explore ocular surface situation and visual quality after small incision lenticule extraction in patients with different degrees of myopia.

Methods

A total of 208 patients with myopia undergoing small incision lenticule extraction in our hospital from January 2022 to December 2024 were enrolled . They were divided into the high myopia group （102 cases，204 eyes） and the low‑to‑moderate myopia group （106 cases，212 eyes）according to the degree of myopia. The uncorrected visual acuity （UCVA），best corrected visual acuity （BCVA），ocular surface situation，visual quality before and after surgery and complications after surgery were compared between the two groups.

Results

The LogMAR values of UCVA and BCVA 1 week，2 weeks，1 month，3 months and 6 months after surgery in the high myopia group were greater than those in the low‑to‑moderate myopia group （

0.05）.The tear meniscus heights of affected eyes 1

week，2 weeks，1 month and 3 months after surgery in the high myopia group were greater than those in the low‑to‑moderate myopia group （

0.05），and scores of lipid layer thickness grading were higher than those in the low‑to‑moderate myopia group （

0.05）.The scores of subjective visual quality questionnaire 1 week and 2 weeks after surgery in the high myopia group were lower than those in the low‑to‑moderate myopia group （

0.05）.No complications such as retinal detachment，glaucoma，keratoconus，corneal opacity or infection occurred in either group within 6 months postoperatively.

Conclusion

Small incision lenticule extraction is effective and safe in the treatment of patients with different degrees of myopia. However，early postoperative ocular surface situation and visual quality in patients with low‑to‑moderate myopia are better than those with high myopia.
