摘要

目的

探讨经腋乳胸锁乳突肌肌间入路腔镜手术治疗单侧甲状腺癌的效果。

方法

选取2020年1月至2023年3月我院收治的单侧甲状腺癌患者116例，患者均行腔镜下单侧甲状腺切除+峡部切除+中央区淋巴结清扫术。按照随机数字表法将患者分为观察组和对照组，每组58例。观察组患者采用经腋乳胸锁乳突肌肌间入路，对照组患者采用经锁骨下入路。比较2组患者手术相关指标、手术前后应激指标[促肾上腺皮质激素（ACTH）、去甲肾上腺素（NE）、皮质醇（Cor）]、甲状旁腺激素（PTH）、血钙水平、术后切口美观满意度及并发症发生情况。

结果

2组患者手术时间、术中出血量、术后引流量比较，差异均无统计学意义（

0.05）；观察组患者中央区淋巴结清扫数目多于对照组（

0.05）。2组患者术后1 d、3 d的NE、Cor、ACTH水平均高于术前（

0.05），且术后1 d的NE、Cor、ACTH水平高于术后3 d（

0.05）；观察组患者术后1 d、3 d的NE、Cor、ACTH水平低于对照组（

0.05）。2组患者术后1 d、3 d的PTH、血钙水平均低于术前（

0.05），术后3 d的PTH、血钙水平低于术后1 d（

0.05），但术后1 d、3 d组间PTH、血钙水平比较差异无统计学意义（

0.05）。观察组患者术后切口美观满意度（100%）高于对照组（72.41%），差异有统计学意义（

0.05）。2组患者并发症发生率比较，差异无统计学意义（

0.05）。

结论

经腋乳胸锁乳突肌肌间入路腔镜手术治疗单侧甲状腺癌安全可行，应激反应轻，且切口隐匿，患者切口美观满意度高。

Abstract

Objective

To explore the effect of endoscopic surgery via trans-axilloareolar sternocleidomastoid intermuscular approach for unilateral thyroid cancer.

Methods

A total of 116 patients with unilateral thyroid cancer admitted to our hospital from January 2020 to March 2023 were selected， and all of them underwent unilateral thyroidectomy+isthmus resection+central lymph node dissection under endoscopy. The patients were divided into the observation group and the control group according to random number table method， with 58 cases in each group. Patients in the observation group received the trans-axilloareolar sternocleidomastoid intermuscular approach，and patients in the control group received subclavian approach. The surgical-related indicators，stress indicators ［adrenocorticotropic hormone （ACTH）， norepinephrine （NE）， cortisol （Cor）］before and after surgery， parathyroid hormone （PTH）， blood calcium level， postoperative incision aesthetics satisfaction， and occurrence of complications were compared between the two groups.

Results

There was no statistically significant difference in the operation time， intraoperative blood loss， or postoperative drainage volume of patients between the two groups （

0.05）. The dissection number of lymph nodes in the central region of patients in the observation group was greater than that in the control group （

0.05）.In the two groups，the levels of NE，Cor，and ACTH 1 day and 3 days after surgery were higher than those before surgery （

0.05）， and the levels of NE， Cor and ACTH 1 day after surgery were higher than those 3 days after surgery （

0.05）； the levels of NE， Cor and ACTH 1 day and 3 days after surgery of patients in the observation group were lower than those in the control group （

0.05）. In the two groups， the levels of PTH and serum calcium 1 day and 3 days after surgery were lower than those before surgery （

0.05）， and the levels of PTH and serum calcium 3 days after surgery were lower than those 1 day after surgery（

0.05）， while there were no significant differences in the levels of PTH or serum calcium 1 day and 3 days after surgery betwe

en the two groups （

0.05）. The satisfaction of postoperative incision aesthetics in the observation group （100%） was higher than that in the control group （72.41%）， with statistically significant difference（

0.05）. No statistically significant difference was found in the incidence of complications between the two groups （

0.05）.

Conclusion

Endoscopic surgery via trans-axilloareolar sternocleidomastoid intermuscular approach is safe and feasible for unilateral thyroid cancer，which has a mild stress response and inconspicuous incisions，with high satisfaction of incision aesthetics for patients.
