摘要

目的

比较常规口腔管理与多维度综合管理在预防清髓性异基因造血干细胞移植（allo-HSCT）患者口腔黏膜炎（OM）中的效果。

方法

回顾性分析2023年1月到2024年12月91例行清髓性allo-HSCT的患者资料，将其分为常规组（

=45）与多维度综合管理组（

=46）。常规组患者实施常规口腔护理；多维度综合管理组患者实施包括基于信息-动机-行为技巧模型的多途径口腔健康维护行为培训、结构化口腔照护、前置抗病毒管理及亚叶酸钙双途径给药解救甲氨蝶呤毒性。比较2组患者OM发生率、严重OM（Ⅲ~Ⅳ级）发生率、愈合时间及OM患者的口腔疼痛分级。

结果

多维度综合管理组OM发生率、OM严重程度、OM平均愈合时间均低/短于常规组（

0.001），OM发生部位数低于常规组（

0.05），OM中位发生时间早于常规组（

0.001）；多维度综合管理组患者口腔疼痛程度低于常规组（

0.05）。

结论

多维度综合管理可有效降低清髓性allo-HSCT患者OM发生率、严重程度及口腔疼痛等级，并缩短其愈合时间。

Abstract

Objective

To compare the efficacy of conventional oral management versus multidimensional comprehensive management in preventing oral mucositis （OM） in patients undergoing myeloablative allogeneic hematopoietic stem cell transplantation （allo-HSCT）.

Methods

A retrospective analysis was conducted on the data of 91 patients who underwent myeloablative allo-HSCT from January 2023 to December 2024. The patients were divided into the conventional group （

=45） and the multidimensional comprehensive management group （

=46）. Patients in the conventional group received conventional oral care， while patients in the multidimensional comprehensive management group received an integrated intervention including multi-channel oral health maintenance behavior training based on the information-motivation-behavioral skills model， structured oral care， preemptive antiviral management， and dual-route calcium folinate administration to counteract methotrexate-induced mucosal toxicity. The incidence of OM， the incidence of severe OM （grades Ⅲ to Ⅳ）， healing time， and oral pain grade in patients with OM were compared between the two groups.

Results

The incidence， severity， and mean healing time of OM in the multidimensional comprehensive management group were lower/shorter than those in the conventional group （

0.001）； the number of involved sites of OM was lower than that in the conventional group （

0.05）； the median OM onset time was earlier than that in the conventional group （

0.001）. The degree of oral pain in the multidimensional comprehensive management group was lower than that in the conventional group （

0.05）.

Conclusion

Multidimensional comprehensive management can effectively reduce the incidence and severity of OM， alleviate oral pain， and shorten the healing time in patients undergoing myeloablative allo-HSCT.
