摘要

目的

探讨盐酸戊乙奎醚在妇科宫腔镜手术中的应用效果及安全性。

方法

回顾性选取于淮北市妇幼保健院2023年1月至2024年3月行宫腔镜手术的80例患者，根据麻醉方案不同分为观察组（

=43）和对照组（

=37）。观察组患者于麻醉诱导前静脉推注盐酸戊乙奎醚，对照组患者麻醉诱导前无处理。比较2组患者麻醉诱导前（T0）、扩张宫颈时（T1）、术中（T2）、手术结束时（T3）、麻醉苏醒时（T4）的心率（HR）、平均动脉压（MAP）、脉搏血氧饱和度(SpO

)等体征指标变化，术中丙泊酚用量、苏醒时间以及不良反应发生率。

结果

观察组患者T1~T2的HR、T1~T3的MAP、T1~T4的SpO

均高于对照组（

0.05）。观察组患者术中丙泊酚用量低于对照组（

0.05），苏醒时间短于对照组（

0.05）。观察组术中窦性心动过缓发生率及麻醉恢复期恶心、呕吐发生率均低于对照组（

0.05）。

结论

盐酸戊乙奎醚应用于妇科宫腔镜手术中，可有效维持患者的体征稳定，降低丙泊酚用量，缩短苏醒时间，且不良反应发生率低，安全性高。

Abstract

Objective

To explore the application effect and safety of penehyclidine hydrochloride in patients undergoing gynecological hysteroscopic surgery.

Methods

A total of 80 patients undergoing hysteroscopic surgery in Maternity＆Child Care Center of Huaibei were retrospectively enrolled from January 2023 to March 2024，who were divided into the observation group（

=43） and the control group（

=37） according to different anesthesia regimens. Patients in the observation group were intravenously injected with penehyclidine hydrochloride before anesthesia induction， while patients in the control group were not given any treatment before anesthesia induction. The changes of vital sign indicators such as heart rate （HR），mean arterial pressure （MAP），and pulse oxygen saturation （SpO

） before anesthesia induction （T0），immediately after cervical dilation （T1），during surgery （T2），at the end of surgery （T3），immediately after anesthesia recovery （T4），and intraoperative dosage of propofol，awaking time and the incidence of adverse reactions were compared between the two groups of patients.

Results

The HR at T1 and T2， MAP at T1 to T3， and SpO

at T1 to T4 of patients in the observation group were all higher than those in the control group （

0.05）. The int

raoperative dosage of propofol of patients in the observation group was lower than that in the control group （

0.05），and awaking time was shorter than that in the control group （

0.05）. The incidence of intraoperative sinus bradycardia and the incidence of nausea and vomiting during anesthesia recovery period in the observation group were lower than those in the control group（

0.05）.

Conclusion

Penehyclidine hydrochloride applied in gynecological hysteroscopic surgery can effectively maintain stability of patients' vital signs，reduce the dosage of propofol， shorten the awaking time，with low incidence of adverse reactions and high safety.
