摘要

目的

评价基于快速康复理念的个体化口服碳水化合物用于腹腔镜下减重手术患者的效果。

方法

选择行腹腔镜下减重手术的肥胖患者88例为研究对象，随机分为对照组和观察组，每组44例。对照组患者术前常规禁食和禁饮，而观察组患者术前2 h根据理想体重口服4 mL/kg碳水化合物。比较2组患者的反流误吸风险、焦虑和抑郁情绪状况、口渴和饥饿状况、术后恢复质量、胃肠功能恢复时间及术后并发症发生情况。

结果

2组患者麻醉前即

刻的反流误吸风险比较，差异无统计学意义（

0.05）。观察组患者麻醉前的焦虑自评量表（SAS）评分、口渴和饥饿评分均显著低于对照组（

0.05）。观察组患者术后1 d、2 d的恢复质量15量表（QoR-15）评分均显著高于对照组（

0.05）。观察组患者的术后并发症发生率明显低于对照组（

0.05）。观察组患者的肛门恢复排气时间及肛门恢复排便时间均明显早于对照组（

0.05）。

结论

腹腔镜下减重手术肥胖患者个体化口服碳水化合物不仅未增加反流误吸风险，还能缓解患者的焦虑情绪及口渴和饥饿感受，有利于促进术后快速康复且并发症少。

Abstract

Objective

To evaluate the efficacy of individualized oral carbohydrates based on the concept of enhanced recovery after surgery in patients undergoing laparoscopic bariatric surgery.

Methods

A total of 88 obese patients who underwent laparoscopic bariatric surgery were selected as the study subjects and randomly divided into the control group and the observation group， with 44 cases in each group. Patients in the control group received routine preoperative fasting and fluid restriction， while patients in the observation group received 4 mL/kg of oral carbohydrates according to ideal body weight at 2 hours before surgery. The risk of regurgitation and aspiration， anxiety and depression， thirst and hunger， postoperative recovery quality， gastrointestinal function recovery time， and the incidence of postoperative complications were compared between the two groups of patients.

Results

There was no statistically significant difference in the risk of regurgitation and aspiration immediately before anesthesia between the two groups of patients （

0.05）. The self-rating anxiety scale （SAS） score， thirst and hunger scores before anesthesia of patients in the observation group were significantly lower than those in the control group （

0.05）. The quality of recovery-15 （QoR-15） scores 1 and 2 days after surgery of patients in the observation group were significantly higher than those in the control group （

0.05

）. The incidence of postoperative complications of patients in the observation group was significantly lower than that in the control group （

0.05）. The time to first flatus and the time to first defecation of patients in the observation group was significantly earlier than those in the control group （

0.05）.

Conclusion

Individualized oral carbohydrate in obese patients undergoing laparoscopic bariatric surgery does not increase the risk of regurgitation and aspiration， but alleviates patients' anxiety， thirst and hunger， which is beneficial to promote rapid postoperative recovery with fewer complications.
