探讨部分关节囊切开修复技术在双极股骨头置换术中对老年股骨颈骨折患者髋关节稳定性及临床疗效的影响。
方法

回顾性分析2021年6月至2023年6月我院收治的105例老年股骨颈骨折患者的临床资料，根据手术方式将患者分为关节囊修复组（
n
=55）和关节囊切除组（
n
=50）。比较2组患者髋关节活动度、髋关节Harris评分（HHS）及假体脱位率。
结果

术后12个月，关节囊修复组与关节囊切除组患者髋关节屈曲、外展角度比较，差异均无统计学有意义（

0.05），关节囊修复组患者髋关节外旋角度小于关节囊切除组（

0.05），假体脱位率低于关节囊切除组（

0.05）；术后12个月关节囊修复组患者HHS评分高于关节囊切除组（

结论

双极股骨头置换术中应用部分关节囊切开修复技术可有效减少术后髋关节外旋角度，降低假体脱位率，改善关节功能，缓解疼痛，尤其适用于老年股骨颈骨折患者。该技术通过保留关节囊张力与完整性，为提升术后髋关节稳定性提供了可行策略，具有临床推广价值。
Abstract

To investigate the effects of partial capsulotomy repair technique in bipolar hemiarthroplasty on hip joint stability and clinical outcomes in elderly patients with femoral neck fractures.
Methods

A retrospective analysis was conducted on the clinical data of 105 elderly patients with femoral neck fractures admitted to our hospital from June 2021 to June 2023. The patients were divided into the capsular repair group （
n
=55） and the capsular excision group （
n
=50） based on surgical technique. The hip range of motion， Harris hip score （HHS）， and incidence of prosthetic
dislocation were compared between the two groups.
Results

There was no statistically significant difference in the angles of hip flexion or abduction 12 months after surgery between the capsular repair group and the capsular excision group （
P

0.05）. The hip external rotation angle in the capsular repair group was significantly smaller than that in the capsular excision group （
P
<
0.05）， and the incidence of prosthetic dislocation was lower than that in the capsular excision group （
P
<
0.05）. The HHS score 12 months after surgery in the capsular repair group was significantly higher than that in the capsular excision group（
P
<
0.05）.
Conclusion

The application of partial capsulotomy repair technique in bipolar hemiarthroplasty can effectively reduce the postoperative hip external rotation angle，lower the incidence of the prosthetic dislocation， improve joint function， and alleviate pain， particularly in elderly patients with femoral neck fractures. By preserving capsular tension and integrity， this technique provides a feasible strategy for enhancing postoperative hip stability and holds clinical value for broader application.
